Otologic or structural facial neuropathy#

What supports it. Chronic otitis, cholesteatoma, mastoid findings, parotid mass, trauma, surgery, progressive hearing loss, or recurrent same-side palsy suggests local infection, injury, or tumor.

What argues against it or keeps uncertainty open. A single rapidly evolving classic peripheral episode without mass, ear disease, or recurrence favors idiopathic palsy.

Discriminating next step. Obtain ENT evaluation and targeted temporal-bone, parotid, or brain imaging for progressive, recurrent, traumatic, or otherwise atypical presentations rather than routine imaging of every Bell-palsy pattern.

The differential is ranked but not closed. Probability, consequence of delay, reversibility, and test burden are considered together. A dangerous alternative can deserve early exclusion even when it is not the statistically most likely explanation.

Evidence-gathering strategy#

Tests are selected because they can change a decision, not because a broad panel feels comprehensive. Results are interpreted with their timing, pretest probability, measurement limitations, recent treatment, and the possibility that an apparently reassuring value was obtained too early or under the wrong conditions.

Progressive course and interpretation#
