What supports it. Early severe hirsutism, rapid progression, virilization, family pattern, very high testosterone or dehydroepiandrosterone sulfate, or elevated morning 17-hydroxyprogesterone supports an adrenal or tumor pathway.

What argues against it or keeps uncertainty open. Slow mild acne and hair change with modest androgens and no virilization fits polycystic physiology more than a tumor.

Discriminating next step. Obtain properly timed 17-hydroxyprogesterone and high-quality androgen measurements, then use endocrine testing and pelvic or adrenal imaging when the level and tempo justify it.

The differential is ranked but not closed. Probability, consequence of delay, reversibility, and test burden are considered together. A dangerous alternative can deserve early exclusion even when it is not the statistically most likely explanation.

Evidence-gathering strategy#

Tests are selected because they can change a decision, not because a broad panel feels comprehensive.
