Benign inflammatory biliary stricture#

What supports it. Prior pancreatitis, biliary surgery, chronic inflammatory disease, autoimmune pancreatitis features, or fluctuating narrowing can produce a nonmalignant stricture.

What argues against it or keeps uncertainty open. Older age, progressive weight loss, a growing mass, vascular involvement, or metastatic lesions keep malignancy dominant until adequately evaluated.

Discriminating next step. Integrate imaging pattern, medication and procedural history, selected IgG4 assessment, and properly planned tissue sampling; avoid using a steroid response as an unstructured diagnostic trial.

Intrahepatic cholestatic disease#

What supports it. Drug exposure, hepatitis, infiltrative liver disease, sepsis, or autoimmune cholestasis is more plausible when ducts are not dilated and imaging shows no mechanical level.

What argues against it or keeps uncertainty open. Clear upstream duct dilation and an obstructing mass strongly favor extrahepatic mechanical disease, though concurrent liver injury can worsen laboratory abnormalities.

Discriminating next step. Review medicines and systemic illness, obtain targeted serology and liver imaging, and involve hepatology if obstruction is not demonstrated or cholestasis persists after decompression.

The differential is ranked but not closed. Probability, consequence of delay, reversibility, and test burden are considered together. A dangerous alternative can deserve early exclusion even when it is not the statistically most likely explanation.

Evidence-gathering strategy#

Tests are selected because they can change a decision, not because a broad panel feels comprehensive.
