Idiopathic pulmonary fibrosis#

What supports it. Older age, progressive fibrotic symptoms, a confident usual interstitial pneumonia pattern, and no identified exposure, medicine, or systemic disease support IPF.

What argues against it or keeps uncertainty open. Marked mosaic attenuation, a clear antigen relationship, prominent inflammatory features, or autoimmune manifestations reduce confidence in an idiopathic label.

Discriminating next step. Confirm HRCT pattern and exclusion work in multidisciplinary discussion; reserve biopsy for cases where uncertainty remains important and procedural risk is acceptable.

Drug induced interstitial lung disease#

What supports it. Symptoms and opacities that appear after a medicine known to cause pneumonitis, improve after withdrawal, or recur with exposure support drug toxicity.

What argues against it or keeps uncertainty open. Longstanding stable treatment, a more specific alternative phenotype, and progression despite adequate withdrawal make that medicine a less complete explanation.

Discriminating next step. Create a dose and timing history including oncology, antiarrhythmic, antimicrobial, and immune therapies, stop a plausible culprit with prescriber coordination, and monitor objective recovery.

Infection or cardiogenic edema#

What supports it. Fever, acute sputum change, focal findings, immunosuppression, orthopnea, edema, elevated filling pressures, or rapid radiographic change can mimic or complicate ILD.

What argues against it or keeps uncertainty open. A slow restrictive course with characteristic fibrosis and no infectious or volume features makes these less likely as the sole cause.

Discriminating next step. Use targeted microbiology, echocardiography, natriuretic peptide context, volume assessment, and short-interval imaging when acute reversible disease remains plausible.

The differential is ranked but not closed. Probability, consequence of delay, reversibility, and test burden are considered together. A dangerous alternative can deserve early exclusion even when it is not the statistically most likely explanation.

Evidence-gathering strategy#

Tests are selected because they can change a decision, not because a broad panel feels comprehensive. Results are interpreted with their timing, pretest probability, measurement limitations, recent treatment, and the possibility that an apparently reassuring value was obtained too early or under the wrong conditions.
