Why Guidelines Stopped Recommending Aspirin for Most Healthy Adults

Newer randomized trials showed that in adults without established cardiovascular disease, aspirin's benefit is small and largely offset by extra bleeding. That is why guidelines pulled back.

For most healthy adults, daily aspirin is no longer recommended to prevent a first heart attack or stroke, because a run of large randomized trials found that its benefit in people without established cardiovascular disease is small and is largely canceled out by the extra bleeding it causes. In 2022 the U.S. Preventive Services Task Force advised against starting aspirin for primary prevention at age 60 and older, and framed it as an individual choice for adults aged 40 to 59 with higher cardiovascular risk. Importantly, that reversal applies only to people who have never had an event. For those who already have diagnosed cardiovascular disease, the case for aspirin remains solid.

Key points#

Start with the arithmetic, not the percentage#

A headline like "aspirin cuts heart attacks by a third" sounds decisive, but a relative number is meaningless without the baseline it acts on. Cutting a 1-in-1000 yearly risk by a third prevents about 3 events for every 10,000 people treated in a year. If aspirin also produces a similar number of serious bleeds in those same 10,000 people, the ledger nets close to zero. Some individuals simply trade a clot they would have had for a hemorrhage they would not have had.

This is the core reason the guidance moved. Nothing changed about how aspirin affects platelets. What changed was a clearer accounting of the absolute numbers on both sides of the scale in people at low starting risk.

One pill, two very different questions#

Most confusion about aspirin comes from collapsing two situations into one word.

Secondary prevention applies to someone who already has cardiovascular disease: a prior heart attack, an ischemic stroke, or a coronary stent. Their yearly risk of another event is high, so a treatment that lowers relative risk delivers a large absolute payoff.

Primary prevention applies to someone with no known cardiovascular disease. Their baseline risk is lower, so the same relative benefit yields a much smaller absolute one, while the bleeding harm barely budges. That asymmetry is the whole story.

The Antithrombotic Trialists' Collaboration meta-analysis, published in The Lancet in 2009, quantified this using individual-patient data. In secondary prevention, aspirin cut serious vascular events by roughly a fifth, an absolute benefit of about 1.5 percent per year. In primary prevention, the proportional reduction was smaller, the absolute benefit was on the order of hundredths of a percent per year, and major gastrointestinal and other bleeds outside the skull rose by about half. The authors judged the net value for healthy people to be genuinely uncertain, and that uncertainty is precisely what the next generation of trials set out to settle.

The trials that forced a rethink#

Between the 2016 guidance and the 2022 revision, three large randomized trials reported, and they told a consistent story.

ASCEND enrolled more than 15,000 adults who had diabetes but no known cardiovascular disease. Aspirin did lower serious vascular events, but the reduction was closely matched by a rise in major bleeding, leaving only a marginal net gain.

ARRIVE studied adults judged to be at moderate risk. Their actual event rates came in lower than predicted, and aspirin failed to reduce cardiovascular events while raising gastrointestinal bleeding.

ASPREE tested low-dose aspirin in more than 19,000 community-dwelling adults aged 70 and older who were healthy at entry. Aspirin did not extend disability-free survival and did not meaningfully cut first heart attacks or strokes. It did significantly increase major hemorrhage, and all-cause mortality was slightly higher in the aspirin group, an excess linked mostly to cancer deaths that is still debated. For a drug meant to protect, a mortality signal pointing the wrong way is a serious finding.

None of these trials claimed aspirin does nothing. They showed that in lower-risk people, the balance of benefit against harm no longer favors routine use.

Two forces that tilt the scale further#

Beyond the raw arithmetic, two trends widen the gap between benefit and harm in primary prevention.

First, bleeding harm is not flat across age. The USPSTF notes that the absolute rate of bleeding, and therefore the size of the harm, climbs with age. That is exactly why the 2022 statement draws its firmest line at 60 and older.

Second, background cardiovascular care has improved. Widespread statin use, better blood-pressure control, and lower smoking rates have pushed baseline event rates down. That leaves aspirin less room to help. A drug that looked useful against the higher baseline risk of earlier decades is now being asked to work against a healthier modern one.

What the 2022 recommendation actually says#

The USPSTF statement is more precise than most headlines suggest. For adults aged 40 to 59 whose 10-year cardiovascular risk is 10 percent or higher, starting low-dose aspirin is called an individual decision (Grade C), meaning any net benefit is small and hinges on personal factors, including bleeding risk and preference. For adults 60 and older, it recommends against starting aspirin for primary prevention (Grade D). That is a clear step down from the 2016 guidance, which had given a Grade B endorsement to part of this group.

Two boundaries are worth keeping in view. The statement addresses starting aspirin, not a universal instruction to stop it, and it explicitly leaves out anyone with elevated bleeding risk. It is a framework for a shared decision, not a one-size rule.

Established disease is a different calculation#

For people who already have cardiovascular disease, the evidence did not weaken and the guidance was not reversed. Their high baseline risk means the same antiplatelet effect prevents far more events in absolute terms, so the benefit continues to outweigh the bleeding cost for most. This is the situation captured by the secondary-prevention data in the ATT analysis and reaffirmed in cardiology guidance.

The takeaway is not that aspirin is a weak drug. It is that the identical drug lands differently depending on who takes it. Reading the evidence well means matching the treatment to the person's absolute risk, which is exactly what good preventive care is meant to do.

The 2022 guidance is about whether to start, not a blanket instruction to stop. Whether to continue depends on your own history, your bleeding risk, and whether you have established cardiovascular disease. That is a conversation to have with your own clinician rather than a decision to make from a guideline headline.

Does this change anything for someone who had a heart attack or a stent?

No. The pullback applies to primary prevention. For people with established cardiovascular disease, the benefit of aspirin still outweighs the bleeding risk for most, and that guidance was not reversed.

Why did advice that stood for decades reverse so suddenly?

It was not sudden so much as cumulative.
