Evaluating Low-T Claims: How Testosterone Deficiency Is Actually Diagnosed in the Evidence

A defensible low testosterone diagnosis pairs matching symptoms with a genuinely low reading confirmed on two separate fasting morning draws. A single convenient number is not the same thing.

A genuine diagnosis of testosterone deficiency is never a single number. The 2018 Endocrine Society clinical practice guideline reserves the diagnosis for men who have both symptoms and signs of deficiency and a testosterone level that is unequivocally, repeatedly low. Both halves have to line up, and the low reading has to hold up when it is checked again on a different morning. A convenient result drawn at a random hour, in a man who feels well, is not that.

Understanding this one distinction is enough to read most "Low-T" advertising with a clear head. The sections below walk through what the evidence asks a clinician to do, then hand you the same questions to test a claim against.

The direct-to-consumer model tends to collapse a careful process into a single visit. A checklist of vague complaints, one blood draw taken whenever you happen to walk in, and a same-day verdict can convert a normal afternoon dip into a "diagnosis." Every shortcut in that funnel removes one of the safeguards the guideline builds in: the morning timing that accounts for the daily rhythm, the fasting condition, the second confirmatory draw, and the rule that the number and the symptoms must agree. What survives is a figure with none of the context that gives it meaning. Marketing is not obliged to explain any of this, which is exactly why it helps to know what a rigorous workup would have done instead.

What a rigorous workup does, step by step#

It starts in the morning, fasting#

The first measurement is total testosterone from a fasting, morning blood sample, run on a reliable assay. The timing is not fussiness. Testosterone rises and falls on a daily cycle and sits highest in the early hours, so a level drawn in the afternoon can look low in a man whose morning value is perfectly normal. A recent meal and a passing illness can drag the number down too, which is why fasting and morning conditions are written into the recommendation.

It confirms before it concludes#

If your first morning value is low, the guideline asks for a repeat fasting morning measurement before anyone attaches a diagnosis. The reason is concrete: an individual's testosterone swings from one day to the next, and about 30 percent of men whose first reading lands in the low range test normal when the draw is repeated. Treating a single low result as settled would misclassify roughly one in three of them. A careful evaluation treats the first number as the opening of an inquiry, not its close.

It reads the number against the right yardstick#

The guideline ties the biochemistry to a harmonized reference range built from healthy young men, with the central 95 percent of that group falling between roughly 264 and 916 ng/dL. Ranges shift with the assay and the laboratory, so a value only carries meaning alongside the method that produced it. A generic cutoff printed on a marketing page is not the same as the range your reporting laboratory uses.

It weighs symptoms as half the diagnosis, not a formality#

Symptoms are not garnish on a lab value. In the guideline's logic they carry equal weight. The relevant complaints, reduced libido, fewer morning erections, flagging energy, or shrinking muscle mass, overlap so heavily with sleep deprivation, depression, thyroid disease, medication side effects, and normal aging that none of them points to testosterone on its own. A low number with no matching clinical picture, or a convincing symptom story with a normal confirmed level, does not meet the standard. The evidence wants both arrows aimed the same way.

It slows down in the borderline zone#

Total testosterone can deceive when sex hormone-binding globulin runs high or low, which happens with obesity, diabetes, thyroid disease, and aging. When a man's total value sits near the lower edge of normal, or he has a condition that shifts that binding protein, the guideline recommends estimating free testosterone by equilibrium dialysis or a validated calculation. A borderline total is a signal to measure more carefully, not to prescribe more quickly.

Because the guideline is really a sequence, you can turn it into a short checklist and run any advertisement or clinic pitch through it:

Why the 2025 label change is not a green light#

In February 2025 the FDA announced class-wide labeling changes for testosterone products, and two of them drew attention. The agency removed the boxed-warning language about increased cardiovascular risk and added a summary of the TRAVERSE trial, a randomized study of more than 5,000 middle-aged and older men who had hypogonadism plus elevated cardiovascular risk, published in the New England Journal of Medicine in 2023. That trial found testosterone therapy did not raise the rate of major adverse cardiac events compared with placebo. The same trial reported more nonfatal arrhythmias, including atrial fibrillation, and the FDA added blood-pressure warnings drawn from required post-market studies. In other words, the update loosened one caution while tightening another.

The reason to be precise about the wording is that a relaxed warning is easy to misread as a broad safety endorsement. Removing cardiovascular language from a boxed warning says the earlier warning was not supported by the trial evidence in the population studied. It does not say the drug is risk-free, and it does not suggest that more men should be treated. The label still carries a limitation of use for age-related hypogonadism, and the products remain indicated only for men with testosterone deficiency from a diagnosed medical condition, not for low readings chalked up to aging alone. The diagnostic threshold the Endocrine Society lays out did not move because a warning was edited. If anything, the new blood-pressure caution and the arrhythmia signal underline that any decision to treat belongs to a person and a clinician weighing that person's specifics, not to an advertising funnel.

The bottom line#

The difference between a rigorous diagnosis and a marketing conclusion is measurable: two mornings against one afternoon, symptoms that fit the biochemistry against a checklist, a confirmed value against a lone number. When a Low-T claim reaches you, the useful move is not to trust it or dismiss it on instinct but to run it through the sequence the evidence describes. A number opens a question. The diagnosis is whatever is still standing after careful repetition.
