Helicobacter Pylori Test and Treat: How the Strategy and 2024 Guideline Work

The 2024 American College of Gastroenterology guideline makes optimized bismuth quadruple therapy first line for most adults, and requires a breath or stool test of cure after every course.

Test and treat for Helicobacter pylori is a simple loop with a strict order: prove there is a live infection, give antibiotics matched to what actually still works in your region, then re-test to prove the bacterium is gone. In September 2024 the American College of Gastroenterology updated the two later steps in a clinical guideline published in the American Journal of Gastroenterology. The headline changes are that optimized bismuth quadruple therapy is now the preferred first-line regimen for most adults, that the old proton pump inhibitor plus clarithromycin combination should no longer be prescribed blind, and that a documented test of cure is expected after every course. One force drives all three changes: medicines that dependably cleared this organism twenty years ago now fail too often to trust on autopilot.

Key points#

Resistance is the reason everything changed#

For close to two decades the standard treatment was a proton pump inhibitor with clarithromycin and amoxicillin for one to two weeks. That regimen only works while the bacterium stays sensitive to clarithromycin, and across much of North America it no longer reliably does. The 2024 guideline notes that resistance to clarithromycin and to levofloxacin has climbed steeply, and that eradication drops to roughly 30 percent once a strain is clarithromycin-resistant. Even so, the clarithromycin triple regimen stayed the most commonly written prescription, which is a mismatch between habit and evidence. The guideline now advises against using it empirically unless susceptibility testing has shown the strain is sensitive, and that testing is rarely done in day-to-day practice. Amoxicillin is the encouraging counterexample: resistance stays in the low single digits, which is part of why amoxicillin-based regimens performed better in trials.

What the guideline puts first#

The preferred regimen for adults who have never been treated, and also for those who already failed a clarithromycin course, is 14 days of optimized bismuth quadruple therapy. It combines acid suppression with bismuth, tetracycline, and metronidazole. The word "optimized" is doing real work here. It signals that each drug is dosed at full strength for the full two weeks, because underdosing the antibiotics or stopping early is exactly where earlier bismuth regimens lost their edge. When the resistance profile of a patient's strain is unknown, this is the guideline's default empiric choice.

Four medicines several times a day is a heavy schedule, so the guideline also names alternatives for patients who cannot manage or tolerate the quadruple regimen. Rifabutin-based triple therapy is one. Regimens built around a potassium-competitive acid blocker are another; that newer class, which includes vonoprazan, holds stomach pH higher and steadier than a traditional proton pump inhibitor, which can help the antibiotics work. These are descriptions of approved treatment categories rather than endorsements of any single product. The right pick depends on drug allergies, what antibiotics you have taken before, and how well you tolerate each option.

Picking the right test, at both ends#

The "test" half of the strategy only counts if the assay detects a living infection rather than an old one. The urea breath test and the stool antigen test both do this well, with high sensitivity and specificity, and both can be repeated afterward to confirm cure. Antibody blood tests behave differently and are the common trap. They can stay positive for months or years after the bacterium is eradicated, so a positive result cannot tell a current infection apart from a resolved one, and false positives rise where infection is uncommon. For that reason the guideline points clinicians toward breath or stool testing whenever the real question is whether someone is infected right now.

The strategy shows up most often in the workup of uninvestigated indigestion. The 2017 ACG and Canadian Association of Gastroenterology dyspepsia guideline recommends that patients younger than 60 with no alarm features be tested noninvasively for H. pylori and treated if positive, with endoscopy held back for older patients or those with warning signs. Two shortcuts defeat the whole approach: testing with no intention to treat, and treating without ever confirming the result.

Confirming the cure is now mandatory#

The most consequential procedural shift is the final step. The guideline states that eradication should be confirmed in every treated patient, using a stool antigen test, a urea breath test, or a gastric biopsy. Timing is where this goes wrong most often. Re-testing too early yields false negatives, so it should wait until at least four weeks after antibiotics are finished and at least two weeks after stopping any proton pump inhibitor or potassium-competitive acid blocker, because acid suppression can blunt the assay and hide a lingering infection. If you need relief during that gap you can usually rely on H2-receptor blockers or antacids, which do not interfere the same way. Serology has no role at this stage either, since it will not turn negative even after a genuine cure.

Who should be tested in the first place#

The 2024 guideline also broadens who warrants testing and treatment. Beyond an active peptic ulcer, it supports testing people at higher risk of gastric cancer, those with atrophic gastritis or gastric intestinal metaplasia, and household members of an infected adult, all with non-serologic tests. That widening reflects the standing of H. pylori as a World Health Organization Class I carcinogen and its causal link to gastric mucosa-associated lymphoid tissue lymphoma. Clearing the infection can lower gastric cancer risk and lets an ulcer heal.

Questions and answers

Can a blood test tell me if I still have H. pylori after treatment?

No. Antibody levels can stay high long after the bacterium is cleared, so a blood test cannot confirm cure. Use a stool antigen or urea breath test, and time it correctly.

How long after finishing antibiotics should the test of cure be done?

At least four weeks after the last antibiotic dose, and at least two weeks after stopping a proton pump inhibitor or potassium-competitive acid blocker. Earlier testing risks a falsely negative result.

Why is clarithromycin no longer the automatic first choice?

Resistance has risen to the point that eradication falls to about 30 percent in resistant strains. Unless a lab has confirmed the strain is susceptible, the guideline steers away from prescribing it blind. The thread running through every step is stewardship. Matching the regimen to real-world resistance, finishing a full optimized course, and verifying the result all protect two things at once: the individual patient in front of you and the dwindling set of antibiotics that still work against this organism.
