IBD Versus IBS: How the Two Are Told Apart

IBD and IBS can cause identical daily symptoms, but only IBD produces measurable gut inflammation. A stool test for fecal calprotectin tells them apart and can spare you a colonoscopy.

Two people can describe the same week of cramping, urgency, and unpredictable stools, and one may have inflammatory bowel disease (IBD) while the other has irritable bowel syndrome (IBS). The fastest non-invasive way to tell them apart is a stool test for fecal calprotectin, a protein that leaks into the gut only when the intestinal lining is actively inflamed. A low value points toward IBS and can spare you a colonoscopy; a high value flags inflammation worth looking at directly. What that single number means, however, is not fixed: it shifts with the cutoff the lab uses and with how likely the disease was before the test was ordered.

Why the two conditions look alike#

At the bedside, symptoms are a poor referee. Weight loss, blood in the stool, symptoms that wake a person at night, anemia, and a strong family history all tilt the odds toward IBD. Yet early Crohn's disease can look mild and unremarkable, and plenty of people with IBS carry alarming-sounding complaints that turn out to be functional. The core difference is invisible from the outside: IBD is defined by chronic inflammation a camera and biopsy can see, while IBS is diagnosed by its symptom pattern and by the absence of a structural cause. That is exactly why a test that reports on the tissue itself, rather than on how you feel, earns its place in the workup.

The stool test that reads the gut wall#

Calprotectin is a calcium-binding protein that fills neutrophils, the white blood cells the body rushes to inflamed tissue. When those neutrophils move into an inflamed intestinal wall and are shed into the bowel, they carry calprotectin with them, and it appears in stool roughly in proportion to how much inflammation is present. Because IBS does not recruit this wave of neutrophils, its calprotectin stays low. A practical bonus, described by Walsham and Sherwood in their 2016 review in Clinical and Experimental Gastroenterology, is that the protein holds up in a stool sample for days, so you can collect the sample at home and mail it to the lab. The threshold most manufacturers still print, 50 micrograms per gram, comes from early hospital work and has become the default line in the sand.

How well the test performs#

A triage test proves its worth mainly by ruling disease out. In a 2010 meta-analysis in the BMJ, van Rheenen and colleagues pooled diagnostic studies and reported roughly 93 percent sensitivity and 96 percent specificity for calprotectin in detecting IBD in adults. High sensitivity is the property that matters most here, because it means very few real cases are missed, so a negative result can be trusted. Their modeling suggested that screening with calprotectin before deciding on endoscopy could cut the number of adults sent for the procedure by about two thirds, while missing roughly 6 percent of IBD cases. That is a deliberate trade, not a cost-free win.

Moving the cutoff moves the errors#

The 50 microgram line is a convention, not a law of nature. Set it lower and sensitivity rises while specificity drops, so fewer cases escape but more healthy people are flagged for scopes they did not need. Set it higher, as some labs do at 100 or 150, and the errors flip the other way. The systematic review and economic evaluation led by Waugh for the UK Health Technology Assessment program found that in adults, laboratory calprotectin separated IBD from IBS with about 93 percent sensitivity and 94 percent specificity at the 50 cutoff, while individual studies scattered from 83 to 100 percent sensitivity and 60 to 100 percent specificity. That spread is the honest picture. If your result lands in the grey zone between clearly normal and clearly abnormal, the usual next step is a repeat test or closer assessment rather than a snap verdict.

The result only means something alongside the patient's risk#

A test does not hand you an answer; it updates a probability that already existed. In primary care, where most abdominal complaints are functional and IBD is uncommon, a negative calprotectin is powerfully reassuring, because the disease was unlikely from the start and the test's high sensitivity pushes the remaining odds close to zero. The identical negative result in a young patient with weight loss, rectal bleeding, and anemia deserves far less reassurance, because the starting risk was high enough that no single stool marker can wave it away. This is why predictive values shift with the setting. In a hospital series by Fu and colleagues in Scientific Reports in 2017, calprotectin at the 50 cutoff showed about 76 percent sensitivity and 93 percent specificity, with a positive predictive value near 97 percent but a negative predictive value of only about 53 percent, precisely because that population was loaded with genuine disease. Same test, very different meaning, depending on who is being tested.

What a high value cannot prove#

Calprotectin marks inflammation, not IBD in particular. It also climbs with intestinal infections, with regular use of nonsteroidal anti-inflammatory drugs, with some polyps and cancers, and it behaves less predictably in very young children. So a single raised value says "something inflammatory is going on, look closer," not "this is Crohn's disease." Walsham and Sherwood note that the test performs less sharply in unselected primary care than in specialist clinics, around 82 percent sensitivity and 77 percent specificity in one large primary care cohort, another reminder that context shapes the answer. Calprotectin sorts patients; colonoscopy and biopsy still make the diagnosis.

Putting it together into a workup#

The modern approach is layered rather than reliant on any one step. Your symptoms and warning features set the initial level of suspicion. Calprotectin then sorts the ambiguous middle: a clearly low value in a low-risk person supports a functional diagnosis and avoids an invasive test, while a clearly high value, or any high-risk presentation regardless of the marker, earns a direct look inside. Each layer covers the blind spots of the one before it.

Questions and answers

Can a normal calprotectin rule out IBD completely?

Not by itself. A low value is very reassuring when the baseline risk was already low, but in someone with red-flag features such as bleeding, weight loss, or anemia, a normal result should not close the case. Risk and result are weighed together.

Does a high calprotectin mean I have Crohn's disease or colitis?

No. It signals active inflammation, which IBD can cause but so can infections, certain medications, polyps, and other conditions. A raised value is a reason to investigate further, usually with colonoscopy and biopsy, not a diagnosis on its own.

Why do different labs report different cutoffs?

The threshold is a design choice that balances missed cases against false alarms. Lowering it catches more disease but flags more healthy people; raising it does the reverse. Labs pick the cutoff that fits their patient population and testing strategy.
