Most moles are harmless and stay about the same for years. Melanoma is set apart less by any single look than by a spot that is changing, or that clearly stands out from your other moles. You often cannot be certain by looking, so the useful skill is not naming a spot but noticing which one deserves a professional look, and acting on it promptly.

Almost everyone has moles. Count the ones on a forearm and you will usually find several, and the great majority of them will never cause any trouble at all. Melanoma is far less common, but it is the skin cancer that most warrants attention, because it can spread if it is not caught in time. That gap, common and harmless on one side, uncommon and serious on the other, is exactly what makes the question feel loaded when a spot catches your eye in the mirror.

The goal of this post is narrow and intended to be calming: to help you recognize the kind of change on your skin that deserves a professional look, and to explain why the timing of that look matters. No article can diagnose a spot on your body, and nothing here is meant to replace a clinician who can actually examine it.

What an ordinary mole is#

A common (benign) mole is usually unremarkable in a good way. It tends to be round or oval, a single even color (tan, brown, or close to your skin tone), with a smooth, clean border. Most are smaller than a pencil eraser, roughly 6 mm across. Many people first notice moles in childhood and early adulthood, and having a good number of them is entirely normal. The American Academy of Dermatology and the American Cancer Society describe the typical benign mole in much these terms.

The most reassuring feature of a benign mole is not on this list, though. It is stability. An ordinary mole looks about the same month after month and year after year. It does not suddenly change its outline, gain a new color, or start to itch.

There is a wrinkle worth naming. Some people have atypical (dysplastic) moles that look irregular, larger, or more mottled than a textbook mole, and yet are still benign. An unusual-looking mole is not, by itself, a diagnosis. This is precisely why appearance alone is not enough, and why change over time turns out to be the more useful signal than any single snapshot.

The ABCDE rule, explained plainly#

The ABCDE rule is a memory aid for the features that make a mole worth showing to a clinician. Each letter is one prompt:

The ABCDE framework comes from the American Academy of Dermatology, and it also appears in the clinical literature on how melanoma is evaluated (for example, a 2025 review of diagnostic techniques, PMID 40581423). One point deserves emphasis, because it is easy to lose. ABCDE is a set of prompts to seek evaluation. It is not a scoring system that confirms or excludes cancer. A spot can tick several boxes and be benign, and, less often, a melanoma can be small and subtle. The letters tell you when to ask, not what the answer is.

The ugly duckling sign and the value of change over time#

Here is a pattern that turns out to be surprisingly useful. A given person's moles tend to resemble one another, sharing a rough family look in size and color. The ugly duckling sign takes advantage of that: a spot that clearly stands out from its neighbors, the odd one out, deserves attention even if it does not tick every ABCDE box. Your own moles are the reference set, and the mole that does not fit the set is the one to flag. The American Cancer Society describes both the ABCDE rule and the ugly duckling sign as everyday tools for noticing melanoma.

Change over time is often the strongest everyday clue of all. A spot you can compare against how it looked a few months ago is far more informative than one you are seeing for the first time. This is why simple comparison photos help: a phone picture with a coin or ruler beside a spot gives you a baseline to check against later, and it gives a clinician something concrete to work from.

It is worth being plain about the evidence here. Studies of these visual cues are largely observational, or focus on how well members of the public apply them rather than on whether they change outcomes. A 2017 study of skin self-examination, for instance, looked at whether pattern-focused visuals help people perform a self-check (PMID 28759333). Work of that kind can tell us these cues guide attention; it does not turn them into a home diagnostic test. They are prompts, not proof.

Why early detection changes outcomes#

The reason all of this matters comes down to a fairly consistent pattern: melanoma caught while it is still thin and localized is highly treatable, and how a case tends to go is strongly tied to the stage at which it is found. The American Cancer Society tracks survival by stage, and the difference between melanoma confined to the skin and melanoma that has spread is large. Localized disease found early carries a very favorable outlook at the population level.

Two caveats keep those numbers in proportion. First, those figures describe populations, not any single person; they are stage-dependent, drawn from large groups, and updated periodically as data mature. No statistic can promise how one individual's case will go. Second, this is a reason for prompt evaluation, not a reason for alarm. The useful takeaway is not fear but timing: noticing a change early, and acting on it, is what gives the best chance of a straightforward path. A spot examined this month is almost always easier to deal with than the same spot ignored for a year.

How to check your own skin, and when to see a clinician#

A skin self-check does not need to be elaborate. A workable routine looks like this:

Some findings should move you from watching to booking a visit. See a clinician for a spot that itches, bleeds, crusts, will not heal, changes in any way, or clearly stands out from your other moles. None of these guarantees a problem; each is a reasonable reason to ask.

Risk is a matter of degree, and it shapes how attentive to be. People with many moles, a personal history of melanoma, a family history, fair skin, or significant past sun-related skin damage may benefit from a clinician's guidance on how often to be checked and whether periodic professional skin exams make sense for them. On population screening of people without symptoms, the US Preventive Services Task Force has weighed the evidence and set out where it stands; that is a statement about screening the general public, not a reason to sit on a spot that worries you. Whatever your risk, the same limit applies to all of us at home: only a clinician, and if needed a biopsy, can actually diagnose a lesion.

Know your skin, and act on the odd one out#

Picture a small brown spot on your shoulder that has been there for years, and next to your memory of it, the same spot now with a slightly ragged edge and a darker patch it did not have last summer. That contrast, not the spot in isolation, is the thing to act on. At a visit, a clinician may examine your skin, sometimes use dermoscopy (a magnified look at the skin surface), and, if a lesion looks suspicious, take a biopsy that is sent to pathology, which is the definitive step. Many spots that look worrying turn out to be benign, and asking about one is never an overreaction. Learn your own skin, watch for change and for the mole that does not match the others, and get a changing or concerning spot evaluated promptly, because with melanoma, early action is what counts most.
