Because there are no symptoms, this happens invisibly. A woman can feel entirely well while her bone density is dropping. The loss only announces itself if a bone gives way, most often at the wrist, spine, or hip. Spinal changes can be subtle at first: losing height over a few years, or a gradual rounding of the upper back, can reflect small compression fractures in the vertebrae that occurred without a dramatic injury.

It is important to be clear that this is a normal change, not a sentence. Knowing that the transition is the high-loss window is exactly what makes it a good moment to think about measurement and prevention, rather than something to fear.

Who should be screened, and when#

Here the guidance is refreshingly concrete. The US Preventive Services Task Force recommends bone density screening for all women 65 and older, and for postmenopausal women younger than 65 who are at increased fracture risk based on a formal clinical risk assessment. Both are B recommendations, meaning the Task Force judges the net benefit to be moderate to substantial (USPSTF osteoporosis screening recommendation).

The under-65 group is where judgment comes in. Being postmenopausal is not, by itself, a reason to screen at 55; the decision rests on whether your estimated fracture risk is high enough to make the test worthwhile now rather than later.

A note on scope: for screening men, the same review found insufficient evidence to recommend for or against it (an I statement), which is why this article stays focused on women. And because the right timing depends on your individual risk, the sensible move is to raise it with your own clinician rather than book a scan on your own. They can tell you whether you fall into the "screen now" or the "reassess later" group.

How risk is estimated before or alongside screening#

For the under-65 question, clinicians lean on formal risk-assessment tools, the best known being FRAX. FRAX estimates the 10-year probability of a major fracture from factors that each nudge risk up or down: age, low body weight, a prior fragility fracture, a parent's hip fracture, current smoking, higher alcohol intake, rheumatoid arthritis, and long-term steroid use, among others.

Two uses are worth separating. Before screening, a tool like FRAX helps identify which younger postmenopausal women carry enough risk to justify a scan now. Alongside screening, it helps interpret a borderline density result by placing it in the context of everything else about you.

What these tools are not is a self-test verdict. A number from an online calculator is an input to a conversation, not a diagnosis. The point of the assessment is to support a shared decision between you and your clinician about testing and, later, about treatment.

What the bone density test is actually like#

If screening is recommended, the test itself is undramatic. DXA (dual-energy X-ray absorptiometry, sometimes written DEXA) is a short scan of the hip and spine. You lie still on a padded table while an arm passes over you. There are no needles, and the radiation dose is very low, lower than many routine imaging tests. Usually the only preparation is to hold off on certain supplements the day of the scan if your center asks, per local instructions.

The result comes back as a T-score, which compares your bone density to that of a healthy young adult. In broad terms:

It is one input, not a standalone diagnosis; your clinician reads it together with your fracture-risk factors, and two women with the same number can warrant different plans. What actually protects bone#

The evidence-supported basics are unglamorous and they work. In general terms:

One point cuts against a common instinct. For healthy postmenopausal women, the USPSTF recommends against routine low-dose calcium plus vitamin D taken solely to prevent fractures (USPSTF, JAMA 2018). More is not automatically better. The goal is to meet your needs, ideally through diet, not to reflexively add pills.

When medication enters the picture#

For women who screen into the osteoporosis range or are otherwise at high fracture risk, lifestyle measures remain the foundation, but they are usually not the whole plan. Several prescription therapies have been shown to reduce fractures, and guideline groups frame the choice around individual fracture risk, patient preference, and the balance of benefits and harms for that person.
