The short answer#
Cellulitis is a bacterial infection of the skin and the soft tissue just beneath it, and clinicians sort it into two groups before treating. Nonpurulent cellulitis has no pus and no abscess, is usually driven by streptococci, and is treated with an oral antibiotic that covers strep. Purulent infection means pus is present as an abscess or a boil; it is usually staph, often MRSA, and the main treatment is draining the pus, with antibiotics added when the infection is larger or the person is systemically unwell. Most cases are handled at home. Spreading redness with fever, or pain far worse than the skin looks, is a reason for you to be seen the same day.

What is cellulitis and how do clinicians recognize it?#
Cellulitis is an acute bacterial infection of the deeper skin layer (dermis) and the fat beneath it. It shows up as four local signs: warmth, redness, swelling, and tenderness. StatPearls notes that two of those four are enough to make the working diagnosis, and there is no blood test or scan that confirms it. The diagnosis is clinical.
A few features help. Cellulitis is usually on one side, most often a lower leg, and its border tends to be fuzzy rather than sharply drawn. That contrasts with erysipelas, a more superficial cousin with a raised, bright red, well-demarcated edge. Bacteria usually get in through a break in the skin, so clinicians look for an entry point: cracked skin between your toes from athlete's foot, a cut, an ulcer, or a bite. If someone marks the leading edge of the redness with a pen, you can see over the next day whether it is advancing or retreating.

What is the difference between purulent and nonpurulent skin infection?#
The 2014 Infectious Diseases Society of America guideline built its whole treatment map on one question asked first: is there pus? That answer predicts the likely organism and the main lever of treatment.

The guideline is blunt about wording: the term "cellulitis" is not appropriate for inflammation sitting over a collection of pus, because the fix for pus is to let it out. Within each track, severity is then graded mild, moderate, or severe, which decides oral versus intravenous drugs and clinic versus hospital.
How is cellulitis treated?#
For nonpurulent cellulitis that is mild, meaning no fever or other systemic signs, the target is streptococci. Reasonable oral options include cephalexin or penicillin, with clindamycin for those who cannot take a beta-lactam. When systemic signs appear (fever, fast heart rate), the same infection is graded moderate and often needs intravenous antibiotics and closer watching.
For purulent infection, drainage leads. A cutaneous abscess is treated by incision and drainage, and the IDSA guideline notes that mild, drained abscesses may need no antibiotic at all, even some caused by MRSA. When antibiotics are added for a larger or more severe purulent infection, they aim at Staphylococcus aureus, including MRSA, with oral options such as trimethoprim-sulfamethoxazole, doxycycline, or clindamycin. Duration is shorter than you might expect. The IDSA guideline recommends five days for uncomplicated cellulitis and extending only if the infection has not improved, and American Family Physician summarizes trial data showing five days worked as well as ten. It is worth being honest about the evidence base. A Cochrane review of trials in cellulitis and erysipelas found mostly small studies and could not name a single best regimen; it even found oral antibiotics performing at least as well as intravenous ones in several comparisons. In other words, the framework above is sound, but the fine print is guided by modest data and clinical judgment.
What conditions mimic cellulitis, such as stasis dermatitis?#
A striking number of "cellulitis" cases are something else. Reviewed in American Family Physician, about 30 percent of people admitted for lower-extremity cellulitis are misdiagnosed, which drives avoidable hospital stays and antibiotics.
Stasis dermatitis is the classic impostor. It comes from chronic venous congestion, tends to be bilateral, itches more than it hurts, and leaves brownish skin staining above the inner ankle. That bilateral pattern is the tell: as DermNet puts it, cellulitis of the lower legs is almost always on one side, so redness on both legs at once should prompt a rethink before another antibiotic course. Other mimics include deep vein thrombosis, contact dermatitis, gout, superficial thrombophlebitis, lipodermatosclerosis, and a drug-induced pseudocellulitis seen with certain chemotherapies.
To bring some structure to the guesswork, the ALT-70 rule scores asymmetry, a raised white cell count, a fast heart rate, and age 70 or older; higher scores make true cellulitis more likely and lower scores argue for a mimic. It is a nudge, not a verdict, and has not been validated in primary care. When does a skin infection need the hospital?#
Some findings should short-circuit the clinic visit and send you for urgent, in-person assessment. The most feared is necrotizing infection, a surgical emergency in which infection races through the tissue planes. Warning signs, drawn from StatPearls and the necrotizing-infection literature, include pain far out of proportion to how the skin looks, redness that spreads fast, skin turning dusky, purple, or black, blood-filled blisters, a crackling feel under the skin, new numbness, and a person who looks toxic with high fever, confusion, or low blood pressure. These cannot be watched at home.
Short of that, reasons for you to be seen the same day or to consider hospital care include a fever with shaking chills, red streaking running up the limb, redness that keeps expanding after about 48 hours on the right antibiotic, involvement of the face or the area around the eye, an animal or human bite, and infection in someone with diabetes, poor circulation, or a weakened immune system. When you are in doubt, having a clinician draw the border and reassess is safer than waiting it out.
It cannot diagnose or treat a specific infection. Anyone with a spreading, painful, or feverish skin infection should contact a clinician or urgent care.
