In cirrhosis, the danger usually announces itself through pressure rather than pain. Blood that should drain easily from the gut through the liver meets a scarred, resistant organ, backs up, and raises the pressure in the portal system. That single mechanism drives the complications people fear most: veins that swell and bleed, fluid that pools in the abdomen, and episodes of confusion. The 2022 Baveno VII consensus, published in the Journal of Hepatology, matters because it lets a clinician estimate that pressure from two ordinary test results, a liver stiffness reading and a platelet count, instead of threading a catheter into a vein.

The measurement that stayed out of reach#
The reference standard for portal pressure is the hepatic venous pressure gradient (HVPG), the difference between a wedged and a free hepatic vein. Baveno VII draws its main line at an HVPG of 10 mmHg. At or above that threshold, the odds of developing varices, of a first decompensating event, and of dying from liver disease all start to rise, which is why the consensus labels this level clinically significant portal hypertension.
The catch was never the concept but the logistics. Obtaining an HVPG means transjugular catheterization at a center equipped to do it, so in practice most people with chronic liver disease never had the measurement. Risk was estimated at a distance, from imaging and lab trends, with no direct read on the pressure that actually mattered.
Two numbers most patients already have#
Baveno VII replaces that gap with a pair of noninvasive rules built on liver stiffness by transient elastography and the platelet count.
- Rule out. A liver stiffness of 15 kPa or lower together with a platelet count of at least 150 x 10^9/L makes clinically significant portal hypertension unlikely, with sensitivity and negative predictive value above 90 percent.
- Rule in. A liver stiffness of 25 kPa or higher is enough on its own to establish it, with specificity and positive predictive value above 90 percent.
The consensus also fills in some of the space between. A stiffness of 20 to 25 kPa with platelets under 150, or 15 to 20 kPa with platelets under 110, likewise points toward significant portal hypertension. Think of stiffness and platelets as two independent witnesses to the same underlying process: scar tissue stiffens the liver, while a congested spleen and a struggling marrow pull the platelet count down. When both witnesses agree, a catheter rarely adds anything.
What remains is a genuine grey zone. Plenty of patients sit between the confident rule-out and rule-in bands, and for them one elastography number does not settle the question. Here Baveno VII points to further tools, spleen stiffness measurement in particular, and reserves the HVPG for cases where it is available and would actually change the plan, rather than sending everyone back to an invasive procedure.
A rule of five for prognosis#
Once portal hypertension is on the table, the next question is how worried you should be. Baveno VII answers with a deliberately plain mnemonic, a rule of five: liver stiffness values of 10, 15, 20, and 25 kPa mark rungs of steadily higher risk of decompensation and liver-related death. The point of keeping it round is usability. A single elastography value maps onto a rough prognostic tier at the bedside with no arithmetic. Two patients can both technically carry the label of advanced liver disease, one at 12 kPa and one at 24 kPa, yet the framework makes plain that their near-term risk is not the same.
Retiring the word cirrhosis at the bedside#
Baveno VII also carries forward a subtle relabeling. It prefers "compensated advanced chronic liver disease" (cACLD) to "compensated cirrhosis." The reasoning is that severe fibrosis and early cirrhosis blur into each other and cannot be split cleanly at the bedside, or often even on biopsy. What guides care is the risk of portal hypertension and decompensation, not the exact histology. Stiffness thresholds anchor the term: a value of 10 kPa or below effectively excludes cACLD, 10 to 15 kPa is suggestive, and above 15 kPa is highly suggestive.
The endoscopy some patients can skip#
The same two results feed a decision Baveno VII inherited from its predecessor. When liver stiffness is under 20 kPa and the platelet count is above 150,000, the chance of high-risk varices is low enough that screening endoscopy can reasonably be deferred and simply repeated over time. For you, that can mean avoiding a procedure you were unlikely to benefit from, which is a large part of why the criteria spread so quickly into everyday practice.
Where the shortcut bends#
The upside is that risk stratification, once tethered to a procedure available in a handful of centers, now begins with an elastography probe and a routine blood draw, putting it within reach of ordinary clinics. The limits deserve equal billing. A 2025 individual patient data meta-analysis in the Journal of Hepatology confirmed that the rule-in and rule-out thresholds hold up well against HVPG, but also showed that their accuracy slips in people with obesity and in metabolic dysfunction-associated steatotic liver disease, prompting proposed adjustments for those groups. And the criteria only sort patients into risk bands. They do not, by themselves, dictate treatment; whether and how to head off decompensation stays an individual clinical judgment.
