The short answer#
Restless legs syndrome (RLS) is an urge to move the legs, usually with an uncomfortable sensation, that is worse at rest and in the evening and eases when you move. It is diagnosed from the story, not a scan. Iron does help many people: a shortage of iron in the brain is a central driver, so clinicians check ferritin and transferrin saturation and correct low stores first. When a drug is needed, current guidance favors gabapentinoids (gabapentin, gabapentin enacarbil, pregabalin) and treats long-term dopamine agonists as a caution, because those medicines can gradually make RLS worse.

What is restless legs syndrome, and what causes it?#
Diagnosis rests on five features, all of which should be present. There is an urge to move the legs, usually with an unpleasant sensation often described as crawling, pulling, or aching. The urge is worse at rest, such as sitting or lying down. It is at least partly relieved by movement like walking or stretching. It is worse in the evening or at night. And it is not better explained by another problem, since leg cramps, positional discomfort, arthritis, and neuropathy can all imitate it. Meeting the story-based criteria matters more than any test.
RLS is common, though how common depends on how strictly you define it. Some form of RLS affects roughly 5 to 15 percent of adults, more often with older age and in women. Clinically significant disease, meaning moderate to severe symptoms that occur at least twice a week and cause distress, is less common, in the range of 2 to 3 percent. Prevalence does rise with age, with upper published estimates reaching roughly 10 to 20 percent in older adults. Many people with RLS also have periodic limb movements during sleep, brief repetitive leg jerks that a bed partner may notice.
Most cases are primary, often with a family history, and reflect two interacting problems in the brain: limited iron availability and altered dopamine signaling. RLS can also be secondary to a recognizable cause. Iron deficiency is the classic one. Pregnancy is another, affecting a meaningful minority of pregnancies, and symptoms usually settle after delivery. Advanced kidney disease raises the risk substantially. Several drug classes provoke or worsen RLS, including many antidepressants, sedating antihistamines, and dopamine-blocking agents used for nausea or psychosis. Does iron really help restless legs, and when is it worth checking ferritin?#
For many people, yes. The brain can be short of iron even when routine blood tests and hemoglobin look normal, so treatment is guided by iron stores rather than by hemoglobin alone. That is why clinicians order a fasting morning ferritin (a storage marker) and transferrin saturation. One caveat: ferritin rises with inflammation, infection, and other stress, so a normal or high number does not always rule out a functional shortage. Oral iron, usually ferrous sulfate, is a reasonable first step when stores are low. Taking it with vitamin C may aid absorption, and alternate-day dosing can be gentler on the gut with similar uptake. When oral iron is not tolerated or not enough, intravenous iron is an option, and the 2025 guideline gives a strong recommendation to intravenous ferric carboxymaltose for suitable patients. Repletion does not work for everyone, but the potential to reduce symptoms with a low-risk, correctable problem is the reason iron comes first. What can I do without medication?#
Nonmedication steps are low-risk and worth starting early, even though the evidence behind them is modest.
Start with a medication review. Ask your clinician whether anything you take could be provoking symptoms, but do not stop an antidepressant or other prescription on your own. Cut back on alcohol, caffeine, and nicotine, especially in the second half of the day. Regular moderate daytime exercise that uses the legs tends to help, while very intense late-evening workouts can backfire for some people. Stretching before bed, a warm or cool soak, leg massage, and pneumatic or vibrating devices give some people short-term relief.

How is restless legs treated with medication, and why the caution about dopamine agonists?#
When symptoms are frequent and disrupt sleep or daily life despite iron correction and lifestyle measures, medication is reasonable. The 2025 guideline gives strong recommendations to the alpha-2-delta ligands: gabapentin enacarbil, gabapentin, and pregabalin. These reduce symptoms, help sleep, and, importantly, do not cause augmentation. Their trade-offs include drowsiness, dizziness, and weight gain, they need dose adjustment in kidney disease, and they add to the sedation of opioids or alcohol.
The bigger shift is what happened to dopamine agonists. Pramipexole, ropinirole, and rotigotine were first-line for years and work well at first. The problem is augmentation: over months to years, the drug itself drives the condition to worsen. Symptoms start earlier in the day, spread from the legs to the arms or trunk, grow more intense, and return faster after a dose. The instinctive fix, raising the dose, tends to accelerate the cycle. If you already take one and are doing well, that is a conversation to have, not a reason to stop suddenly. Abrupt withdrawal can trigger a severe flare, so any change should be a planned taper or switch guided by your clinician. When to seek care#
Some patterns are not typical RLS and deserve prompt attention rather than a sleep aid.
- Sudden numbness, weakness, foot drop, or symptoms fixed to one leg. That points toward a nerve, spine, or circulation problem, not RLS.
- A painful, swollen, warm, or red calf, which can signal a blood clot and needs urgent assessment.
- New or worsening leg symptoms after starting a medication, so the drug can be reviewed.
- Signs of augmentation on a dopamine agonist: symptoms creeping earlier into the day, spreading upward, or needing ever-higher doses.
- New impulse-control changes, such as compulsive gambling or shopping, while on a dopamine drug.
- Severe symptoms in pregnancy, or in a child, where dosing and safety differ and warrant tailored advice.
- Sleep loss heavy enough to affect mood, safety, or daytime function.
