Understanding Carpal Tunnel Syndrome

Carpal tunnel syndrome is a specific median-nerve problem, not a label for every painful or tingling hand. Which fingers, how fast, and how weak decide what evaluation is useful.

Your tingling fingers can come from the wrist, the elbow, or the neck. They can come from a generalized nerve disorder, or a problem that is not primarily neurological. Carpal tunnel syndrome names one particular mechanism: compression of the median nerve as it passes through a fixed passage at the palm side of the wrist.

That definition matters. A familiar label can become too broad, leading every numb hand to be treated with a wrist splint or every abnormal nerve test to be treated as the whole diagnosis. A careful assessment joins anatomy, symptom pattern, and examination. It joins severity and the decision that testing is meant to inform.

A crowded tunnel, a vulnerable nerve#

The carpal tunnel has carpal bones as its floor and sides and the transverse carpal ligament as its roof, and the median nerve and nine flexor tendons pass through this space. The boundaries do not expand much, so swelling of tendon coverings, altered wrist geometry, or other space-occupying changes can raise pressure.

A nerve needs both mechanical integrity and an adequate blood supply. Sustained compression can disturb local circulation, impair signal conduction, and eventually damage myelin and axons. Early disease often produces intermittent sensory symptoms. More advanced disease can produce persistent numbness, loss of fine control, and weakness of median-innervated thumb muscles.

The word “tunnel” can encourage an overly simple picture in which one visible object pinches the nerve. Most idiopathic cases do not have a single removable mass. Pressure reflects the relationship among tunnel size, tissue volume, wrist position, fluid balance, and nerve susceptibility, and a ganglion, fracture deformity, inflammatory tissue, or other structural lesion is possible, but it is not the default explanation.

The sensory map is highly informative#

The median nerve supplies sensation to the thumb, index finger, middle finger, and the thumb-side half of the ring finger. You may notice tingling, burning, or buzzing. You may notice numbness or a swollen feeling even when your fingers do not look swollen. Symptoms commonly wake people from sleep, and shaking the hand or hanging it down can provide temporary relief.

The pattern is not always drawn with anatomical precision. Symptoms may spread across the hand or travel into the forearm, and pain can be more diffuse than numbness. Still, little-finger symptoms should prompt consideration of the ulnar nerve, while symptoms extending from the neck with neck movement may suggest a cervical source.

Sensation over the central thenar eminence can remain normal because the median nerve's palmar cutaneous branch usually leaves the nerve before it enters the tunnel; this is a useful anatomical clue, not an absolute rule. Human descriptions vary, and overlapping nerve supply prevents any single patch of skin from acting as a perfect test.

Motor findings change urgency#

The recurrent motor branch of the median nerve supplies muscles that help move the thumb away from the palm and oppose it toward the fingers. People with advanced compression may drop objects, struggle with buttons, or notice loss of pinch precision. The fleshy thenar area at the thumb base may flatten.

Intermittent nighttime tingling without weakness is different from constant sensory loss with thenar atrophy; the latter pattern raises concern that axonal injury has developed and that complete recovery may be less likely even after pressure is relieved. A normal-looking hand does not exclude carpal tunnel syndrome, but objective weakness or wasting should not be managed as routine wrist discomfort.

Sudden severe symptoms after trauma, rapidly progressive weakness, or major hand swelling require a broader urgent assessment. So do vascular changes or signs of infection. Carpal tunnel syndrome is usually gradual, but acute pressure can occur after fracture, bleeding, or marked swelling.

Why risk factors are not diagnostic tests#

Carpal tunnel syndrome is associated with pregnancy, diabetes, and hypothyroidism. It is associated with inflammatory arthritis, obesity, kidney failure, and prior wrist injury. Symptoms may also occur with repetitive force, vibration, sustained wrist flexion or extension, and work that combines these loads.

An association does not prove the cause in one hand: many people with diabetes never develop carpal tunnel syndrome, and many people with the syndrome do not have an endocrine disorder. Pregnancy-related fluid changes can provoke symptoms that improve after delivery, but persistent or severe findings still need assessment.

Testing every patient for every associated condition is not automatically useful. Medical history and examination should guide targeted evaluation. For example, generalized symptoms in the feet and hands can suggest polyneuropathy, while inflammatory joint findings may support an arthritis assessment.

Diagnosis begins with pattern and probability#

The 2024 American Academy of Orthopaedic Surgeons guideline gives strong support to using the six-item Carpal Tunnel Syndrome clinical tool, called CTS-6, instead of routinely obtaining ultrasonography or nerve-conduction and electromyography tests. The tool combines characteristic numbness, nighttime symptoms, and thenar weakness or atrophy into a probability-oriented assessment. It also uses sensory change and provocative findings.

This does not mean that a questionnaire replaces clinical reasoning. It means a coherent history and examination can be sufficiently discriminating in a typical case. Tinel's sign, in which tapping over the nerve triggers symptoms, and Phalen-type wrist-flexion maneuvers can contribute. Neither sign is reliable enough to rule the condition in or out alone.

A diagnostic label should also fit the functional story. Which fingers are affected? Is the problem intermittent or constant? Does it wake you? Are there tasks that reveal weakness? Is the little finger involved? Are there neck symptoms, bilateral foot symptoms, joint swelling, trauma, or a mass? Those questions may matter more than repeating a provocative maneuver several times.

When electrical testing or ultrasound adds value#

Nerve-conduction studies measure how electrical signals travel across the wrist. Electromyography samples muscle electrical activity. Together they can support localization, estimate physiological severity, and identify other neuropathies, and they are useful when the clinical probability is uncertain, when severe or atypical findings need characterization, or when the result will change a procedural decision.

An abnormal study is not the same thing as your symptoms. Mild slowing can exist without meaningful symptoms, and a test can be normal in an early clinical case. Interpretation should match the history and should account for technical methods, temperature, comparison nerves, and other neurological conditions.

Ultrasound can measure median-nerve enlargement and assess anatomy. It is noninvasive and may identify a structural cause. Its accuracy depends on technique and threshold, and it does not measure nerve function in the same way as electrodiagnostic testing. The current guideline treats these tools as options selected for a question, not mandatory hurdles before every diagnosis. Plain radiographs and advanced imaging generally address suspected bone injury, arthritis, tumor, or unusual anatomy rather than confirming ordinary carpal tunnel syndrome; a test is most useful when its answer can change what happens to you next.

The main mimics deserve an explicit check#

Cervical radiculopathy can cause arm pain, sensory change, or weakness in a root distribution and may vary with neck position. Ulnar neuropathy commonly affects the little finger and the other half of the ring finger. Generalized polyneuropathy often begins distally in the feet and later affects both hands.

Thumb-base arthritis and flexor-tendon disorders can cause hand pain without median sensory symptoms, and pronator syndrome compresses the median nerve farther up the forearm and may produce forearm discomfort with a different sensory pattern. Neurological disease, vascular disorders, and central causes are less common but important when the pattern does not fit.

More than one problem can coexist. A person can have cervical disease and carpal tunnel syndrome, or diabetic polyneuropathy plus focal median-nerve compression. Finding one plausible abnormality should not end the assessment if important features remain unexplained.

Night splinting is a mechanical trial#

A neutral wrist splint limits flexion and extension during sleep, positions that can raise tunnel pressure; it is commonly used for intermittent mild or moderate symptoms, especially when nighttime waking dominates. Fit matters: a brace that forces your wrist backward, compresses your palm, or sits in a drawer cannot test the intended mechanism.

Improvement supports continued use but does not prove the diagnosis. Lack of improvement can reflect severe disease, an incorrect diagnosis, or poor fit. It can reflect inconsistent use, or expectations that were too rapid. Nonsurgical care should include a planned reassessment rather than indefinite brace replacement.

Activity changes can reduce aggravating force or sustained wrist posture. The goal is practical load modification, not a claim that all keyboard use or every repeated motion causes the syndrome. Evidence for many exercises, manual techniques, oral medicines, and device variations is less consistent than promotional language may imply.

Corticosteroid injection buys time, not certainty#

Injection into the carpal tunnel can reduce symptoms for weeks or months. In the pragmatic INSTinCTS trial of mild or moderate disease, injection produced better symptom and function scores at six weeks than a night splint. Other placebo-controlled evidence also shows short-term benefit and possible delay of surgery.

Duration is the limitation. The 2024 guideline concludes that corticosteroid injection does not provide long-term improvement. Repeated temporary responses should prompt a discussion of the diagnosis, severity, procedural risks, and more durable options rather than an assumption that repeated injections repair the nerve.

Injection also has potential harms, including pain, skin or fat changes, and temporary glucose elevation. Others are infection and rare nerve or tendon injury. Image guidance, drug choice, technique, and individual medical factors belong in a clinician's decision. A response to injection can be informative, but it is not a flawless diagnostic test.

What release surgery changes#

Carpal tunnel release divides the transverse carpal ligament to enlarge the tunnel and reduce median-nerve pressure. Open and endoscopic approaches use different access methods but share that mechanical goal. Current evidence does not support one approach as universally best for every patient and surgeon.

The randomized comparison by Gerritsen and colleagues found better outcomes with open release than with splinting, although many participants assigned to splinting later underwent surgery. This illustrates both the greater durability of decompression and the difficulty of interpreting long follow-up after treatment crossover.

Surgery is often discussed when symptoms remain function-limiting despite appropriate nonsurgical care, when a patient prefers a durable option, or when constant numbness, weakness, atrophy, or severe physiological findings suggest advanced compression. Recovery depends partly on how long and how severely the nerve was impaired: your night symptoms may improve quickly, while dense numbness or muscle weakness can recover slowly or incompletely.

Risks include infection, scar tenderness, and pillar pain. They include stiffness, nerve injury, and incomplete release. They include persistent symptoms from another diagnosis, and recurrence. Shared decision-making should compare these risks with the risk of ongoing nerve dysfunction and the burden your symptoms already carry.

A useful follow-up plan has thresholds#

For a mild presentation, a plan might pair neutral night splinting with a defined follow-up interval and clear measures: nighttime waking, sensory episodes, task difficulty, objective strength, and any progression toward constant numbness. The plan should state what would trigger testing, injection, or surgical referral.

For uncertain or atypical symptoms, the next action may be diagnostic clarification rather than another treatment trial. For weakness, muscle loss, or persistent sensory deficit, a more prompt specialist and physiological assessment is reasonable because time can matter to nerve recovery.

The central principle is proportionality. A familiar intermittent pattern does not require every available test. A severe or discordant pattern should not be reduced to a brace recommendation. The history, the examination, and the severity should converge on the same next decision. So should the alternative diagnoses and your own priorities.
