A coronary calcium score measures calcified plaque in the walls of the coronary arteries. The score is direct evidence that coronary atherosclerosis has developed, but it is not a percentage blockage, a forecast of when a heart attack will occur, or a complete inventory of every plaque. Its strongest clinical role is selective risk reclassification when a preventive treatment decision remains uncertain.
The 2026 ACC/AHA multisociety dyslipidemia guideline expanded the role of coronary artery calcium, or CAC, within primary prevention.[1] It now sits after calculation and personalization of risk: estimate risk with the PREVENT-ASCVD equations, consider features the calculator does not capture fully, then use CAC selectively when the result could change treatment or its intensity.

What the scanner sees and how the number is built#
A dedicated CAC scan is a low-dose CT acquired without contrast dye and timed to the electrocardiogram to reduce motion. Calcium appears bright. Software identifies areas in the coronary arteries above a density threshold, and the Agatston method multiplies lesion area by a density factor. Scores from the major coronary vessels are added into one total.
The number therefore reflects both how much calcified area is visible and how dense it is. It is measured in Agatston units, commonly abbreviated AU. It does not state that an artery is a certain percentage blocked. A small calcified plaque could sit in a vessel without causing important narrowing, while a noncalcified plaque could narrow a vessel without adding to the calcium total.
The scan also does not show blood flow under stress. Coronary CT angiography, stress imaging, exercise testing, and invasive angiography answer different questions. A calcium score is a marker of plaque burden and future risk, not a map of ischemia or an acute diagnostic pathway.
Technical factors affect the result. Scanner settings, motion, body size, image reconstruction, heart rhythm, and scoring method can change small measurements. A score of 0 versus a very low nonzero score can sometimes hinge on detection limits. The overall categories and clinical context usually matter more than treating a tiny numerical change as biological certainty.
What common CAC score ranges mean#
Score categories are descriptive rather than universal treatment commands. A common framework is:
- 0 AU: no detectable calcified coronary plaque.
- 1 to 99 AU: mild calcified plaque burden.
- 100 to 299 AU: moderate burden.
- 300 AU or more: high burden.
- 1,000 AU or more: very high burden in observational cohorts.
Risk rises continuously. The boundaries are useful communication points, not cliffs. A score of 99 and 100 do not describe fundamentally different biology. The same absolute score can also carry different implications at different ages. Fifty Agatston units may be unusual for a younger adult and commoner later in life.
That is why your report may give both the absolute score and a percentile based on age, sex, and a reference population. The 2026 guideline emphasizes prognostic value from both.[1] The absolute score describes current burden; the percentile asks how unusual that burden is among people like you. Reference categories that include race can describe observed population patterns, but race is a social classification and an imperfect proxy for biology. The result still requires an individual clinical interpretation.
Why a score of zero can lower estimated risk#
In asymptomatic primary-prevention cohorts, adults with CAC 0 have had substantially lower event rates than adults with detectable calcium. This “power of zero” can reclassify someone whose risk estimate was near a treatment threshold. It may support deferring lipid-lowering medication in a low- or intermediate-risk adult without another condition that already warrants treatment, followed by continued risk-factor management and later reassessment.[1]
The phrase “warranty period” is sometimes used for the lower-risk interval after CAC 0. It should not be taken literally. The length of that interval varies with your age, sex, diabetes, smoking, family history, cholesterol burden, and other factors. Plaque can develop after the scan, and cardiovascular events can occur without detectable calcium at baseline.
Zero means no calcified plaque was detected. Early atherosclerosis can be noncalcified, particularly in younger adults. Acute coronary syndromes often arise from biologically active plaque that a calcium-only scan does not characterize. CAC 0 is therefore more informative for preventive risk stratification in selected middle-aged or older adults than as reassurance for a young person with a strong risk pattern or current symptoms.
The zero result should not override established treatment indications. The 2026 guideline recommends primary-prevention lipid-lowering therapy for adults aged 40 to 75 with diabetes, stage 3 or 4 chronic kidney disease, or HIV, regardless of LDL cholesterol level.[1] Severe hypercholesterolemia, known atherosclerotic disease, and some other high-risk circumstances also require their own pathway. A scan should not be ordered merely to create permission to ignore a clear indication.
Why scores above 100 usually strengthen the case for treatment#
Detectable calcium proves that calcified coronary plaque exists. As burden rises, observed rates of myocardial infarction and other atherosclerotic events rise. Multi-Ethnic Study of Atherosclerosis analyses have shown graded risk across CAC categories and improved risk discrimination beyond conventional factors.[3]
A score of 100 AU or more, or a high percentile for age and sex, often moves an uncertain preventive decision toward lipid-lowering therapy. The 2026 guideline uses CAC to guide both LDL and non-HDL cholesterol goals in appropriate adults.[1] The result can also sharpen attention to smoking cessation, blood pressure, physical activity, diabetes management, and other modifiable risks.
Very high scores identify a high-risk group, but they do not automatically mean a person has already had a clinical coronary event. A 2024 staging proposal suggested aligning more intensive prevention with progressively higher CAC categories, including scores of 300 and 1,000.[5] That article is a proposal and call to action, not a substitute for the guideline or individualized assessment. Observational risk similarity does not make primary and secondary prevention identical.
CAC also does not determine whether aspirin is appropriate. Preventive aspirin decisions involve age, bleeding risk, cardiovascular risk, and current guideline recommendations. A high calcium score can be part of a specialist discussion, but it should not be converted into a medication decision you make on your own.
The 2026 risk framework changed the surrounding calculation#
Older U.S. discussions often placed CAC after the Pooled Cohort Equations and cited the 2018 cholesterol guideline. The 2026 dyslipidemia guideline retired and replaced that document.[1] It recommends PREVENT-ASCVD equations for 10-year and 30-year risk assessment in adults aged 30 to 79, followed by personalization and possible CAC reclassification.
In the new framework, LDL-lowering treatment can be considered at a 10-year PREVENT-ASCVD risk of 3 to less than 5 percent and should be considered at 5 to less than 10 percent after a clinician-patient discussion.[1] CAC can help when treatment or treatment intensity remains unclear. The guideline notes that CAC scoring can improve assessment in men age 40 or older and women age 45 or older, with selection based on whether the result can change management.
This does not mean everyone at those ages should be scanned. A test adds value only when plausible results lead to different reasonable actions. If medication is clearly indicated for you regardless of CAC, the scan may be redundant. If risk is very low and neither CAC 0 nor a modest positive score would change the plan, testing may add little.
Why population screening remains a separate question#
Professional guidelines can recommend selective testing within a risk discussion while a preventive-services panel remains uncertain about screening an entire asymptomatic population. These positions answer different questions.
The current final USPSTF statement, issued in 2018 and under active update, concludes that evidence is insufficient to determine the balance of benefit and harm from adding CAC to traditional risk assessment for asymptomatic adults to prevent cardiovascular events.[2] The Task Force found improved discrimination and reclassification but inadequate evidence that CAC-guided treatment decisions reduce events or mortality. It classified direct test harms as small while noting incidental findings, downstream testing, inappropriate reclassification, and anxiety. The 2026 cardiology guideline incorporates a newer risk equation and later evidence, and it supports selective CAC use.[1] Until the USPSTF update is completed, keep the dates, populations, outcomes, and institutional questions apart rather than reading one statement as though the other did not exist.
What the scan cannot tell you#
It cannot rule out noncalcified plaque#
Calcium generally appears as plaque evolves. Younger plaque may be noncalcified. CAC 0 is reassuring in the right preventive population, but it is not proof that your arteries are anatomically pristine.
It cannot locate the next culprit lesion#
Risk relates to total burden and biology. The plaque with the most calcium is not necessarily the plaque most likely to cause a future event. A score does not predict a date, vessel, or certainty of heart attack.
It cannot measure stenosis#
Agatston units are not percent narrowing. Symptoms suggesting angina or an acute coronary syndrome need evaluation based on presentation and dedicated diagnostic pathways.
It cannot prove that CAC-guided care caused better outcomes#
CAC predicts risk and can reclassify treatment decisions. The causal question is whether a strategy of scanning, interpreting, treating, and following patients improves health compared with a strategy without scanning. That whole pathway requires outcome evidence, not just prediction.
It cannot serve as a simple treatment-response meter#
Calcification can increase over time even as plaque biology changes with lipid lowering. Statin therapy may promote denser calcification as part of plaque stabilization. A rising score does not map cleanly to treatment failure, and a stable score does not prove success. Cholesterol response, adherence, blood pressure, smoking, diabetes, symptoms, and overall risk are more direct management targets.
Test burdens and downstream consequences#
Radiation from modern CAC scanning is usually low, often around the order of 1 millisievert, but dose varies with equipment and body size. The scan may reveal findings outside the coronary arteries, such as lung nodules or aortic abnormalities. Some are clinically important; others lead to repeat imaging, procedures, cost, and worry without benefit.
False reassurance is another harm. CAC 0 can be misread as permission to smoke, stop treating blood pressure, or disregard severe cholesterol. A high score can create panic or prompt tests that are not indicated in an asymptomatic person. The value comes from interpreting the score within a predefined decision, not from collecting a number in isolation.
Cost and access also matter. Insurance coverage varies. If you are going to have the scan, know beforehand how each plausible result would change the plan. “We will decide after seeing it” is less disciplined than agreeing on the decision branches first.
When a CAC discussion is most useful#
Consider the test when all of these are true:
- The person is asymptomatic and the question is primary prevention.
- Baseline risk has been calculated with a current method and personalized with relevant factors.
- A lipid-lowering decision or its intensity remains uncertain.
- Both a score of zero and a clearly positive score could change that decision.
- The person understands radiation, incidental findings, cost, and the limits of zero.
- No separate condition already determines treatment.
The scan is generally not the starting point for chest pain, established atherosclerotic cardiovascular disease, or a clear high-risk indication. It is also less informative when age makes calcification unlikely despite a strong lifetime risk pattern.

The number is evidence, not an instruction#
Read your CAC score as one measured feature of atherosclerotic burden. Zero can lower near-term risk in selected adults, and higher scores strengthen concern, but neither replaces the full risk profile. The right question is not “Is this score good or bad?” It is “What decision was uncertain before the scan, and does this result change that decision under current guidance?”
