Joint pain does not automatically call for a scan. Many common problems can be diagnosed provisionally from the history and physical examination, followed by a time-limited treatment plan and reassessment; imaging becomes valuable when it answers a specific question that could change what happens next.
That question might be “Is there a fracture after this injury?”, “Has osteoarthritis progressed enough to explain the limitation?”, “Is a tendon torn?”, or “Is there a collection that should be aspirated?” The best first test depends on the suspected tissue, acuity, patient characteristics, and consequences of missing the diagnosis.
Ordering the most detailed scan first can create delay, cost, incidental findings, and confusion. Withholding imaging despite red flags can also cause harm. The goal is a proportionate pathway.
Start with the clinical question#
Useful imaging begins with a working differential diagnosis. Location, mechanism, onset, duration, swelling, locking, instability, morning stiffness, systemic symptoms, prior injury, surgery, medicines, and functional loss narrow the possibilities; examination adds alignment, range of motion, tenderness, effusion, strength, stability, skin findings, pulses, and neurologic status.
Say what you suspect on the request. “Knee pain, rule out everything” is less useful than “twisting injury with inability to fully extend; assess occult fracture first, then meniscal or loose-body pathology if radiographs are negative.” A clear question helps the radiologist choose views, protocol, and whether contrast is justified. Ask what result would change treatment; if both positive and negative scans lead to the same conservative plan, immediate imaging may have little value unless a serious alternative must be excluded.
Red flags change the pace#
A visibly deformed joint after trauma, an absent pulse, or a cool limb requires emergency assessment. So does new major numbness or weakness, open injury, compartment-syndrome concern, or inability to move the limb. Imaging must not delay reduction, vascular evaluation, antibiotics, or surgical consultation when those are urgent.
A rapidly hot, swollen, painful joint can be septic arthritis. Risk rises with fever, immune suppression, or older age. It also rises with recent bacteremia, joint replacement, surgery, injection, or skin infection. But absence of fever does not exclude it. Joint aspiration for cell count, crystals, Gram stain, and culture often provides the decisive evidence. Ultrasound can locate an effusion and guide aspiration.
Unexplained weight loss, prior cancer, destructive night pain, or a mass raises concern for malignancy. Persistent fever, severe focal bone pain, and inflammatory markers may suggest osteomyelitis. These paths need targeted urgent evaluation rather than a generic outpatient MRI order.
Acute trauma: decide whether bone imaging is needed#
After an injury, mechanism and ability to bear weight matter, and clinical decision rules such as the Ottawa knee and ankle rules identify patients at low fracture risk who may safely avoid radiographs when every criterion is met. The rule must be applied to the right population. It does not replace your judgment in children, pregnancy, or intoxication. Nor does it replace your judgment in altered sensation or complex trauma.
The ACR acute knee trauma criteria support radiography as initial imaging when a fall or twisting injury produces focal tenderness, effusion, or inability to bear weight. If radiographs show no fracture but an occult fracture or internal derangement remains suspected, MRI without contrast is often the next study.
Dislocation deserves special caution. A knee can spontaneously reduce while leaving popliteal artery injury. Pulse examination, ankle-brachial assessment, serial checks, and CT angiography when indicated can be more urgent than ligament MRI.
Why radiographs are often first#
Plain radiographs show cortical bone, alignment, and dislocation. They show joint-space narrowing, osteophytes, and subchondral sclerosis. They show erosions, calcification, and some effusions. Weight-bearing views can show load-dependent narrowing that a supine MRI does not represent in the same way.
For chronic knee pain, ACR rates knee radiography as the usual initial imaging. The same first-line pattern applies to many chronic shoulder, hip, ankle, hand, and wrist complaints, and a focused set of inexpensive images can establish osteoarthritis or reveal a lesion that changes the next test.
Radiographs use ionizing radiation, but extremity doses are generally low. They do not show cartilage, marrow, ligaments, and many tendons directly. A normal film is a result within a pathway, not proof that no condition exists.
When MRI adds value#
MRI shows marrow, cartilage, menisci, labrum, ligaments, tendons, muscle, synovium, and fluid in detail, and it is useful after negative or inconclusive radiographs when a specific soft-tissue injury, occult fracture, osteonecrosis, marrow lesion, infection, or surgical target remains likely.
For chronic knee pain with normal radiographs or an unexplained effusion, MRI without intravenous contrast is often appropriate. For chronic shoulder pain, MRI or MR arthrography may follow normal films when labral disease or instability is suspected. Ultrasound or MRI may fit suspected rotator-cuff disease depending on local skill and the exact question.
MRI is less useful when known advanced osteoarthritis already explains symptoms and the result will not change care, and degenerative meniscal tears and labral abnormalities are common with age and may not be the pain source. A detailed image can amplify a finding that has weak clinical relevance.
Contrast is a separate decision#
Most routine musculoskeletal MRI for internal derangement uses no intravenous gadolinium. Contrast may help characterize tumor, infection, inflammatory synovitis, postoperative change, or certain complex lesions. MR arthrography injects dilute contrast into a joint for selected labral, cartilage, or instability questions.
“MRI with and without contrast” should not be a default. It adds time, cost, injection burden, and rare adverse effects. Kidney function and prior reactions matter for some agents. Let the protocol follow the pathology you suspect and the radiology guidance.
CT provides excellent cortical-bone detail and can clarify complex fractures, bone loss, or alignment when radiographs are insufficient, though it uses more ionizing radiation than an extremity radiograph and is weaker than MRI for many soft tissues.
Where ultrasound fits#
Musculoskeletal ultrasound provides real-time images without ionizing radiation. It can show joint effusions, synovial thickening, and bursitis. It can show superficial tendon tears, dynamic tendon movement, and some crystal deposits. Doppler can assess active synovial blood flow.
It is also valuable for guiding aspiration, injection, or biopsy while avoiding vessels and nerves. Dynamic maneuvers can reproduce snapping or impingement that a static MRI misses.
Its limits are operator dependence, restricted view through bone, and reduced assessment of deep central structures. A nondiagnostic ultrasound is not equivalent to a normal MRI. Match the modality to the anatomy and to the expertise you can actually get.
Chronic joint pain and osteoarthritis#
Osteoarthritis is a clinical and structural condition, not an imaging score alone. Pain can be severe with modest radiographic change, while marked narrowing can coexist with limited symptoms. Treatment targets function, pain, strength, and participation, not merely the picture.
The ACR chronic hip criteria begin with radiography for undifferentiated chronic hip pain. Hip pain can also come from the lumbar spine, sacroiliac region, tendons, or referred visceral disease. A diagnostic local anesthetic injection can sometimes clarify whether the hip joint is the dominant source when several abnormalities coexist. For chronic ankle pain, current ACR criteria also start with radiographs. MRI or CT may follow for suspected osteoarthritis, impingement, or osteochondral lesion. It may follow for instability or tendon disease, with the choice determined by tissue and prior findings.
Inflammatory and crystal arthritis#
Multiple swollen joints, prolonged morning stiffness, or psoriasis can suggest inflammatory arthritis. So can uveitis, inflammatory bowel disease, or symmetric small-joint symptoms. Radiographs can show distribution, erosions, and chronic damage. Ultrasound and MRI can detect synovitis and erosions earlier in selected settings.
Imaging cannot replace history, examination, inflammatory markers, serology used in context, and specialist assessment. A negative X-ray does not exclude early rheumatoid or psoriatic arthritis.
Gout and calcium pyrophosphate disease may show characteristic findings, but crystal identification from synovial fluid remains highly informative, especially in a first hot swollen joint. A crystal finding also does not exclude simultaneous infection.
Back pain follows a related principle#
The spine is not a peripheral joint, but it demonstrates the same decision rule. The NICE low back guideline advises against routine imaging in a nonspecialist setting for low back pain with or without sciatica and recommends specialist imaging only when the result is likely to change management.
Urgent imaging is appropriate when cauda equina syndrome, infection, cancer, fracture, or severe progressive neurologic deficit is suspected. New bladder retention, saddle sensory loss, or rapidly worsening leg weakness requires emergency assessment.
Disc bulges, degeneration, and facet changes are common in people without pain. Finding them can create fear and unnecessary restriction when they do not match symptoms; the image has to line up with the anatomy and with what you found on examination.
Incidental findings and false certainty#
Every modality can reveal abnormalities unrelated to the complaint. Age-related tendon changes, cartilage defects, cysts, benign bone lesions, and degenerative tears are common. More sensitive imaging increases the chance of detecting them.
Reconcile the report with your examination. A lesion on the opposite side of symptoms or outside the tender structure may not explain the problem. Conversely, a report described as “mild” can still coexist with substantial pain. An image does not measure pain processing, strength, sleep, or function. Avoid both extremes: treating every finding, and dismissing symptoms because imaging looks modest.
A practical decision pathway#
First, screen for emergency and systemic features. Second, identify whether the problem is acute trauma, atraumatic acute swelling, or chronic pain. Third, localize the likely tissue and define what diagnosis would change care.
For trauma with fracture risk, start with appropriate radiographs. For a hot swollen joint, prioritize aspiration and infection assessment, using ultrasound if needed. For chronic native-joint pain, radiographs are commonly first. Use ultrasound for suitable superficial and procedural questions. Reserve MRI or CT for a defined unresolved question.
Finally, decide in advance what you will do with each possible result. Imaging is a decision aid. Its value comes from changing management safely, not from producing the longest report.
