Nonclassic Chest Discomfort in Urgent Care

A burning upper-abdominal sensation, nausea, and shoulder discomfort can still require an acute coronary syndrome pathway. The case turns on triage, serial evidence, and disciplined uncertainty.

Learning objectives#

Initial presentation#

A 61-year-old woman walks into an urgent care clinic late in the afternoon because of a burning pressure beneath the lower sternum and high in the upper abdomen, and it began while she was carrying groceries up one flight of stairs. She stopped, and the discomfort eased after about eight minutes. Two hours later it returned while she was sitting, this time with nausea, sweating, and an ache in the left shoulder, and she calls it bad indigestion and asks whether she can take an antacid and go home.

Her history includes hypertension and a long period of cigarette use that ended three years ago. She takes a blood-pressure medicine but is unsure of its name. She has no known coronary disease. Her mother had a myocardial infarction in her early sixties. The patient prefers to discuss complex information in Spanish, although she can answer basic questions in English.

At triage she is alert. Blood pressure is 154/88 mm Hg, pulse 92 per minute, and respiratory rate 18 per minute. Oxygen saturation is 97 percent on room air, and temperature is normal. The discomfort is now a 3 out of 10. She is not visibly breathless. A brief examination finds clear lungs, regular heart sounds without a new murmur, and a soft abdomen. It finds equal radial pulses and no unilateral leg swelling. Pressing on the chest wall produces mild tenderness but does not reproduce the burning pressure.

The clinic can obtain an ECG, but it cannot run a high-sensitivity cardiac troponin assay or observe serial results.

Problem representation#

This is an adult with cardiovascular risk factors and new exertional then resting central and upper-abdominal pressure accompanied by nausea, sweating, and shoulder discomfort. She is currently stable, but the symptom pattern has progressed over hours; a normal oxygen saturation and a mostly reassuring examination do not lower the risk enough for an antacid trial at home. The immediate problem is possible acute coronary syndrome in a site that cannot complete a validated rule-out pathway. That representation deliberately carries what changes disposition: progression from exertion to rest, associated autonomic symptoms, risk context, and the clinic's testing limits. Mild chest-wall tenderness is recorded, but it is not allowed to erase the rest of the pattern.

Prioritized differential#

1. Acute coronary syndrome#

Reasoning for: The discomfort began with exertion, recurred at rest, and came with nausea and sweating. Upper-abdominal burning and shoulder discomfort can function as anginal equivalents. Hypertension, prior tobacco use, and family history raise the prior probability.

Reasoning against or still uncertain: She is hemodynamically stable and has no known coronary disease. Those facts matter for immediate stability, but neither excludes myocardial ischemia. The diagnosis still requires ECG interpretation, serial cardiac troponin testing, and the clinical course.

2. Gastroesophageal or other upper gastrointestinal pain#

Reasoning for: Burning beneath the sternum and in the upper abdomen can arise from reflux, esophageal spasm, gastritis, or peptic disease.

Reasoning against or still uncertain: Exertional onset, recurrence at rest, and sweating make a purely gastrointestinal explanation less secure. So do nausea and shoulder discomfort. Improvement after an antacid, if it occurred, would not by itself exclude ischemia.

3. Acute aortic syndrome#

Why it cannot be missed: Aortic dissection can be rapidly fatal, and antithrombotic treatment may be harmful if this is the true diagnosis.

Case evidence: There is no abrupt maximal pain, tearing back pain, or pulse deficit. There is no new focal neurologic finding, hypotension, or new aortic-regurgitation murmur. These absences lower concern but do not create certainty. Any new feature in this cluster would redirect the pathway toward urgent aortic imaging and specialist care.

4. Pulmonary embolism#

Why it cannot be missed: Pulmonary embolism can present with chest discomfort, dyspnea, presyncope, or unexplained tachycardia.

Case evidence: She has no pleuritic quality, hypoxemia, or unilateral leg finding. She has no recent immobilization, prior venous thromboembolism, or current hormone use. The present pattern is more suggestive of myocardial ischemia. Pulmonary embolism testing should follow a validated pretest-probability approach if new history or findings raise suspicion, not a reflex scan.

5. Pneumothorax, pericarditis, myocarditis, or an acute abdominal process#

Clear bilateral breath sounds and the absence of sudden pleuritic pain make pneumothorax less likely. The discomfort is not positional, and there is no viral syndrome or typical diffuse ECG pattern yet to suggest pericarditis or myocarditis, and a soft abdomen without focal tenderness reduces concern for an acute abdominal catastrophe. These diagnoses remain conditional alternatives because the examination and ECG may evolve.

6. Musculoskeletal chest-wall pain#

Mild tenderness supports a possible musculoskeletal contributor. It does not reproduce the patient's main symptom and does not explain the exertional and autonomic pattern. Two processes can coexist, so one tender area should not close the case.

Focused history and examination#

The next questions are chosen because each can change risk, testing, or destination.

The focused examination repeats vital signs and assesses appearance, perfusion, and mental status. It assesses heart and lung findings, pulses in both arms, and a focused neurologic screen. It also assesses abdomen and legs. New hypotension, hypoxemia, or pulmonary edema would change urgency and the diagnostic branch. So would a new murmur, pulse asymmetry, or a focal neurologic deficit. A reassuring examination cannot complete an acute coronary syndrome rule-out.

Diagnostic strategy#

At urgent care#

The clinic obtains a 12-lead ECG promptly while arranging emergency medical transport. The tracing shows sinus rhythm, no ST elevation, and subtle horizontal ST-segment depression in the lateral leads. There is no prior tracing for comparison. Because the site cannot perform serial high-sensitivity troponin testing or monitored observation, a normal or nondiagnostic ECG would not have made discharge safe. What you can measure sets what you can exclude. The new depression increases concern and makes transfer more urgent.

Emergency transport is safer than private travel because symptoms can worsen en route and the receiving system can prepare. The handoff includes onset times, symptom progression, and risk factors. It includes vital signs, ECG findings, and medicines and allergies. It also includes language preference and treatments already given. A copy of the ECG travels with the patient.

At emergency care#

The emergency team repeats the ECG and uses its validated high-sensitivity cardiac troponin clinical decision pathway. The pathway must match the exact assay and local timing protocol. An older, less sensitive troponin assay cannot simply be substituted into a rapid pathway designed for a high-sensitivity assay.

Useful initial studies include:

Tests are connected to decisions. A broad computed-tomography survey is not a substitute for a differential. Stress testing or coronary computed-tomography angiography belongs later, if the patient is in an appropriate intermediate-risk branch after myocardial infarction is not established. A single conventional troponin ordered too early is not a rule-out strategy.

Progressive results and interpretation#

The repeat ECG in the emergency department again shows lateral ST depression without ST elevation. Her discomfort resolves, but symptom resolution does not close the episode.

The first high-sensitivity troponin value is below that assay's 99th-percentile upper reference limit. Two hours later, the value is above the reference limit and has risen substantially. Exact numerical deltas are assay-specific, so the team applies the local validated pathway rather than a universal cutoff copied from another laboratory, or from a review article you happen to remember.

The rise and fall of cardiac troponin establishes acute myocardial injury. In this presentation, the ischemic symptom pattern and ischemic ECG change support acute myocardial infarction rather than a nonischemic injury alone. The working diagnosis becomes a non-ST-elevation acute coronary syndrome. Cardiology evaluates the patient through the current acute coronary syndrome pathway.

This sequence illustrates why the first result was not falsely negative. It was a time-limited measurement obtained before the biomarker had risen above the reference boundary. The information became diagnostic only when serial change was combined with the symptoms and ECG.

Later coronary evaluation identifies an obstructive coronary lesion judged responsible for the event. That result confirms the branch chosen earlier, but it does not retroactively make every similar symptom coronary disease. A different patient using the same words could follow another pathway if the ECG, serial troponin pattern, examination, and risk context differed. The words are not the diagnosis.

Management plan#

Immediate actions#

At urgent care, priorities are emergency transport, repeat vital assessment, and cardiac monitoring when available. They also include a clear handoff and avoidance of delay for low-value testing. Antiplatelet therapy may be appropriate in suspected acute coronary syndrome after checking allergy, active bleeding, and features that would raise concern for acute aortic syndrome. Exact treatment follows emergency protocols and is omitted here because dosing and contraindications require patient-specific review.

At the receiving hospital, the team treats the patient through an acute coronary syndrome pathway, consults cardiology, evaluates bleeding and kidney risk, and chooses antithrombotic and invasive strategies based on the confirmed syndrome and current guideline. Oxygen is not a routine treatment for a patient with normal saturation. Symptom relief must not replace disease-directed evaluation.

Alternative branches#

Medication-safety review includes antiplatelet or anticoagulant use, bleeding history, and kidney and liver function. It includes drug interactions and medicines that must be reconciled at discharge. No treatment decision rests on the case analysis alone.

Escalation, referral, and safety net#

This presentation requires emergency transfer at the first site because the condition is time-sensitive and the clinic cannot complete serial evaluation. Escalation becomes immediate for recurrent or severe discomfort, ST elevation, or new arrhythmia. It becomes immediate for fainting, hypotension, hypoxemia, or pulmonary edema. It becomes immediate for new neurologic findings, pulse asymmetry, or a sudden maximal pain pattern.

After hospital care, emergency instructions should be concrete. New pressure, burning, breathlessness, sweating, nausea, fainting, or discomfort in the arm, shoulder, jaw, back, or upper abdomen should prompt emergency services rather than driving or waiting for an office reply. The instruction must be available in the patient's preferred language and confirmed with teach-back.

Communication, shared decisions, and equity#

The clinician begins with a professional medical interpreter rather than asking a family member to translate high-stakes details. A useful explanation is: "Your symptoms can come from the stomach, but the pattern can also happen when the heart is not receiving enough blood. We cannot safely separate those causes here because the answer depends on repeat heart tracings and blood tests over time."

The team avoids describing the symptoms as atypical in a way that means unimportant. Sex, age, race, language, disability, and the patient's own label of indigestion can all distort triage, including yours, if they replace the actual symptom pattern. The same structured questions and escalation thresholds should be applied consistently.

Shared decision-making has a role once immediate threats are addressed, especially for intermediate-risk testing and long-term prevention. It does not turn an incomplete urgent-care evaluation into a preference-sensitive discharge choice. Transportation, caregiving, and cost are part of a safe plan, not social details added after the medical decision. So are health literacy and access to follow-up.

Follow-up and contingencies#

After an acute coronary syndrome, follow-up should reconcile medicines, confirm tolerance and adherence, address blood pressure, lipids, tobacco abstinence, activity, and other modifiable risks, and connect the patient with cardiac rehabilitation. The 2025 guideline identifies secondary prevention and cardiac rehabilitation as central after discharge. Follow-up timing depends on the hospital plan and clinical stability. The clinician also checks whether the patient understood which symptoms require emergency action and whether language, transportation, cost, or pharmacy access could interrupt the plan. If later evaluation contradicts the initial diagnosis, the problem list and patient explanation should be corrected rather than allowing a provisional label to become permanent.

Reasoning traps and alternative pathways#

An alternative pathway remains plausible throughout. If serial troponin values were stable, ECGs nonischemic, and upper gastrointestinal symptoms clearly predominated after a validated cardiac evaluation, reflux or other gastrointestinal disease could move upward. If abrupt back pain, pulse difference, or a neurologic finding appeared, acute aortic syndrome would become the organizing concern. The discipline is to update, not defend the first impression.

Evidence limits and what could change#

Rapid high-sensitivity troponin pathways are assay-specific, and institutions differ in timing, reporting, imaging access, and observation capability. The evidence supports structured pathways, but it does not make every patient interchangeable. Pregnancy, advanced kidney disease, or persistent symptoms can change interpretation. So can prior coronary disease, very early presentation, or an alternative source of myocardial injury.

The 2021 chest pain guideline addresses evaluation, the 2022 ACC pathway details emergency disposition with high-sensitivity troponin, and the 2025 acute coronary syndrome guideline addresses management once the syndrome is established. None is a substitute for local protocols or bedside assessment. The outcome described in this analysis is one plausible branch, not a prediction for you if your own symptoms sound similar.

Does a normal first troponin rule out myocardial infarction?

No. Interpretation depends on symptom timing, the assay, serial change, the ECG, and the rest of the presentation.

Can indigestion-like discomfort be acute coronary syndrome?

Yes. Pressure, burning, upper-abdominal discomfort, breathlessness, nausea, fatigue, and discomfort outside the chest may be ischemic symptoms.

Should a risk score override clinical concern?

No. A validated pathway supports judgment, but instability, ischemic ECG findings, or a strongly concerning presentation requires escalation regardless of a score.
