The link is often missed on both sides: patients do not connect their periods to their fatigue, and the bleeding itself frequently gets normalized as "just how mine are." How iron deficiency and heavy periods are connected#

The mechanism is simple. Blood carries iron, so every period loses a little. Longer or heavier periods lose more. When the monthly loss outpaces what the diet and gut can absorb and replace, iron stores slowly draw down. This is not a sudden event. It is a gradual drift that can take months or years, which is part of why it goes unnoticed.

It helps to think of it as a spectrum rather than an on-off switch. First the body's iron reserves fall (depleted stores). Then iron runs low enough to affect how the body works, but the blood count still looks normal (iron deficiency without anemia). Only later, if the drain continues, does hemoglobin drop and true iron deficiency anemia appear. The reassuring part is that this process is reversible at every stage, and caught early it is corrected before anemia ever develops.

Heavy menstrual bleeding is common, affecting up to roughly a third of reproductive-age women at some point (Munro 2023). Because it is common, it also gets normalized. If your periods have always been heavy, or your mother's were, it is easy to assume that is simply your baseline and nothing to raise. That assumption is precisely what delays recognition.

What counts as a heavy period (and how to gauge it at home)#

There is no need for a lab test to get a first sense of this. A few practical yardsticks tell you a lot:

None of these on its own is a diagnosis, and periods vary. But if one or more sounds like yours, that is worth mentioning to a clinician. Tracking a few cycles helps: jotting down how many days you bleed, how often you change products, and whether you pass clots gives a clinician a much clearer picture than memory alone, and moves the conversation forward faster than saying "they're heavy."

The tone here matters. Heavy bleeding is common and treatable. It is not something to tough out or feel you have to handle on your own, and raising it is not making a fuss. General definitions from ACOG and NIDDK line up closely with the everyday signs above.

The signs of low iron people tend to explain away#

Iron does more than build red blood cells, so low iron shows up in scattered, easily dismissed ways. The classic ones:

The point worth holding onto is that iron deficiency can affect quality of life, energy, and even cognition before anemia shows up on a blood count (Munro 2023). So these symptoms deserve a look rather than a shrug, especially alongside heavy periods, and once identified they are treatable.

When to get tested, and which tests actually help#

A reasonable rule of thumb: if you have heavy or prolonged periods plus fatigue or any of the symptoms above, it is worth asking about iron testing.

Two labs do most of the work, and the distinction between them matters:

This is the crux of the whole topic. A normal hemoglobin is genuinely reassuring about anemia, but it does not rule out iron deficiency. Ferritin is what catches the earlier stage.

One honest caveat: ferritin is an acute-phase reactant, which means it rises with infection or inflammation and can therefore read falsely normal even when stores are truly low. Clinicians account for this by interpreting ferritin in context, sometimes alongside a marker of inflammation such as CRP, or a measure like transferrin saturation. It is not a reason to distrust the test, just a reason it is read with judgment rather than in isolation.

Worth knowing: guidelines have been inconsistent about whether to routinely check iron studies, and not just hemoglobin, in people with heavy bleeding (Mansour 2020). That inconsistency is one reason iron deficiency here is under-diagnosed, and one practical reason to ask specifically about ferritin.

Replacing iron: oral first, IV when needed#

Oral iron is the usual first-line treatment, and a few habits meaningfully improve how well it works. Take it with a source of vitamin C (a glass of orange juice), which aids absorption, and keep it away from tea, coffee, calcium supplements, dairy, and antacids, all of which blunt uptake. Dark stools are expected and harmless. The side effects that actually drive people to quit are nausea and constipation, and it is worth flagging those to a clinician rather than simply stopping, because dose and timing can often be adjusted.

Here the evidence has shifted in a useful direction. Randomized data in iron-depleted women found that a single morning dose taken on alternate days improved the fraction of iron actually absorbed, compared with dividing doses across every day (Stoffel 2017). The reason is a hormone called hepcidin: a large dose of iron raises hepcidin, which then blunts absorption of the next dose given too soon after. Spacing doses out lets hepcidin settle, so more of each dose gets in, often with fewer side effects. The total daily amount to aim for still varies from person to person and should be set with a clinician.

Intravenous iron is the alternative when oral iron is not the right fit. It is generally considered when oral iron is not tolerated or not well absorbed, when bleeding losses are ongoing, when anemia is more significant, or when iron needs to be replaced quickly, for example before a planned surgery (Mansour 2020). It is a shared decision, not a fallback failure.

One detail that surprises people: repletion continues for a while after hemoglobin returns to normal. Correcting the blood count is not the same as refilling the stores, and stopping the moment hemoglobin looks fine tends to leave the tank half full.

Reducing the bleeding itself: the menu of options#

Replacing iron treats the consequence. If heavy bleeding continues, the iron will simply drift down again, which is why addressing the source often prevents recurrence. Treatment frequently works both fronts at once.

A practical path forward: bring it up with a primary care clinician, describe the bleeding in concrete terms (or bring a cycle you have tracked), and ask specifically about checking ferritin, not just a standard blood count. Most iron deficiency from menstrual loss responds well to iron repletion combined, where appropriate, with treatment aimed at the bleeding itself. Follow-up testing later confirms stores have rebuilt and helps decide when to stop. This is a common, well-understood problem that generalist primary care handles all the time.

Can I have iron deficiency if my hemoglobin is normal?

Yes. Iron stores can run low well before hemoglobin drops. That earlier stage, iron deficiency without anemia, can still cause fatigue and other symptoms. A ferritin test reflects iron stores and usually falls before hemoglobin does, which is why checking ferritin, not only a blood count, can matter when periods are heavy.

How heavy is too heavy?

Practical warning signs include soaking a pad or tampon every one to two hours, passing clots larger than a quarter, bleeding longer than seven days, doubling up on protection, or waking overnight to change products.
