Heavy Irregular Uterine Bleeding With Anemia and Anticoagulant Use

Heavy bleeding in a person taking an anticoagulant is both a hemostatic problem and a diagnostic signal. Stabilization, pregnancy exclusion, PALM-COEIN reasoning, cancer-risk assessment, and thrombosis prevention must proceed together.

Learning objectives#

Initial presentation#

Camila, a 46-year-old restaurant manager, requests a same-day visit because she has soaked through pads every one to two hours since the previous evening and passed several clots larger than a coin. When she stood in the shower that morning, she became lightheaded and had to sit on the floor. She feels short of breath climbing one flight of stairs but has no chest pressure or fainting.

For eight months her cycles have become irregular. She sometimes goes six or seven weeks without bleeding, then bleeds heavily for ten days. Between these episodes she has occasional spotting. She assumed this was normal perimenopause. Fatigue, restless legs, and craving ice developed gradually, but she attributed them to night work. She has not had a gynecologic visit in four years.

Ten months earlier, Camila had an unprovoked proximal deep-vein thrombosis and small pulmonary embolism. She takes a direct oral factor Xa inhibitor and took the morning dose before calling. Hematology had planned to revisit extended therapy after a year, but the appointment was rescheduled twice. She also takes medicine for hypertension. She has obesity and a history of irregular cycles since adolescence, without a formal diagnosis of polycystic ovary syndrome. She has never been pregnant, is sexually active with a male partner, and uses condoms inconsistently. She wants to preserve her uterus but is uncertain about future pregnancy.

At the clinic, pulse is 108 per minute and blood pressure is 112/70 while supine. On standing with assistance, she becomes pale and nauseated, pulse rises, and blood pressure falls; the maneuver is stopped. She is alert but anxious. Conjunctivae are pale. Cardiopulmonary examination shows tachycardia without heart failure. The abdomen is soft, with a firm lower-midline fullness but no peritoneal signs. Blood is visible on the pad she changed at arrival. A pelvic examination is deferred until emergency stabilization and consent can be organized.

The clinic arranges emergency transport, establishes the last anticoagulant dose and bleeding timeline, and gives a structured handoff. Camila is not told simply to stop the anticoagulant and call gynecology later. Her hemodynamic symptoms, ongoing loss, anemia risk, and thrombotic history require coordinated care.

Problem representation#

This is a 46-year-old reproductive-aged adult with eight months of irregular, prolonged, and intermenstrual uterine bleeding, now presenting with an acute high-volume episode, orthostatic symptoms, tachycardia, and probable symptomatic anemia while taking a factor Xa inhibitor for a recent unprovoked venous thromboembolism. Obesity and chronic irregular cycles increase concern for ovulatory dysfunction and unopposed endometrial stimulation. Lower-abdominal fullness raises a structural cause such as leiomyoma.

The immediate problem is significant uterine bleeding with impaired orthostatic tolerance. The parallel diagnostic problem is chronic abnormal uterine bleeding that should not be attributed solely to anticoagulation or perimenopause. Pregnancy, structural lesions, endometrial hyperplasia or malignancy, ovulatory dysfunction, and iatrogenic amplification must be evaluated. The anticoagulant plan must account for both current hemostasis and recurrent thrombosis risk.

Prioritized differential#

1. Leiomyoma or endometrial polyp amplified by anticoagulation#

Heavy prolonged bleeding, clots, uterine fullness, and intermenstrual spotting support a structural lesion. A submucosal fibroid or polyp can disrupt the cavity and bleed disproportionately during anticoagulation. Imaging defines structure, while hysteroscopy may be needed for direct evaluation and treatment.

2. Ovulatory dysfunction in the menopausal transition#

Long irregular intervals followed by prolonged bleeding suggest anovulation. Perimenopause is common, but it is a physiologic context, not a diagnosis that excludes pathology. Obesity and long-standing irregular cycles increase cumulative endometrial exposure and affect sampling thresholds.

3. Endometrial hyperplasia or cancer#

Persistent irregular and intermenstrual bleeding, age, obesity, nulliparity, and possible chronic anovulation raise risk. Most patients with these features do not have cancer, but risk is high enough that tissue assessment must be addressed. An anticoagulant can reveal bleeding from an underlying lesion.

4. Anticoagulant-associated bleeding#

The factor Xa inhibitor increases bleeding intensity and may be a major contributor. Drug accumulation from kidney or liver dysfunction, interactions, incorrect dosing, or duplicate antithrombotics could amplify effect. It does not identify the uterine source. Last dose and organ function matter more than routine clotting tests alone.

Pregnancy, early loss, and ectopic pregnancy must be excluded in any patient with pregnancy potential, regardless of age or reported cycle pattern. Camila has no pain or peritoneal sign, but inconsistent contraception means history alone is insufficient.

6. Adenomyosis, endometriosis, or other endometrial disorder#

Adenomyosis commonly causes heavy painful bleeding and a tender bulky uterus; Camila has fullness but little pain. Endometriosis more often produces cyclic pain. Primary endometrial dysfunction remains possible after structural, ovulatory, and systemic causes are assessed.

7. Coagulopathy or systemic disease#

Lifelong heavy bleeding, postpartum or surgical hemorrhage, easy bruising, epistaxis, and family history would suggest inherited bleeding disease. Camila's heavy bleeding is recent, making an acquired or structural cause more likely. Thyroid disease, liver disease, kidney disease, and blood disorders are considered when symptoms or laboratories support them.

Focused history and examination#

Bleeding is described with dates, duration, flooding, pad or tampon frequency, nighttime changes, clots, intermenstrual and postcoital bleeding, pain, pressure, and quality-of-life effects. The goal is not to demand an exact milliliter estimate. Prior cycles, pregnancy history, contraception, fertility goals, cervical screening, previous imaging, procedures, and family gynecologic history are documented.

The team asks about fainting, chest pain, breathlessness, palpitations, exercise decline, pregnancy symptoms, severe pelvic or shoulder pain, fever, discharge, and urinary or bowel pressure. Bleeding history includes menarche, dental and surgical bleeding, bruising, nosebleeds, postpartum hemorrhage, and family history.

Anticoagulant reconciliation records indication, event date, recurrence history, thrombophilia questions only when relevant, last dose, prescribed schedule, missed doses, kidney and liver function, interacting medicines, aspirin, nonsteroidal anti-inflammatory drugs, supplements, and access. The clinician checks whether treatment remains time limited or extended; that decision cannot be reconstructed from memory alone.

Examination reassesses perfusion, orthostatic tolerance when safe, heart, lungs, abdomen, and signs of anemia. With consent and appropriate chaperoning, pelvic examination later assesses active cervical or uterine bleeding, trauma, infection, cervical lesion, uterine size, tenderness, and adnexal findings. The amount on a pad or in the vault is one time point and must be combined with trajectory.

Diagnostic strategy#

Stabilize while identifying pregnancy and blood loss#

Two intravenous access sites are established, blood is typed and screened or crossmatched according to severity, and serial vital signs and bleeding are recorded. Laboratory studies include complete blood count, pregnancy testing, electrolytes, kidney and liver measures, and selected coagulation or drug-specific testing when it can inform anticoagulant effect. Routine prothrombin time or activated partial thromboplastin time cannot reliably quantify every direct oral anticoagulant.

Hemoglobin is 7.2 g/dL with microcytosis, compared with a normal value eighteen months earlier. Ferritin and transferrin studies support iron deficiency. Hemoglobin does not measure the full acute loss before equilibration, so symptoms, perfusion, ongoing bleeding, comorbidity, and trend guide transfusion and resuscitation. Pregnancy testing is negative.

Use structure and risk to choose imaging and tissue assessment#

Transvaginal ultrasound, with transabdominal imaging as needed for uterine size, shows a cavity-distorting submucosal fibroid and a heterogeneous endometrium. Both ovaries are seen without an acute adnexal lesion. Ultrasound identifies likely structure but cannot exclude endometrial hyperplasia or cancer. Endometrial thickness alone is not used as a universal premenopausal cancer cutoff.

Because Camila has persistent irregular and intermenstrual bleeding, age and risk factors, gynecology recommends endometrial assessment. Hysteroscopy offers direct cavity visualization, targeted sampling, and a potential route to treat the submucosal lesion. The plan discusses pain control, setting, alternatives, and consent. A blind sample may miss a focal lesion, while a normal ultrasound would not remove the clinical indication for tissue in this risk context.

Cervical evaluation remains separate from endometrial assessment. The clinician verifies screening history and examines the cervix for a visible lesion, friability, trauma, or infection when Camila is stable and consents. Cervical testing follows screening and symptom-based guidance; a previously normal screening result would not explain away a new visible lesion. Coagulation-disorder testing is not ordered as a routine panel solely because bleeding is heavy. It becomes more relevant if a lifelong bleeding history, family pattern, unusual procedural hemorrhage, thrombocytopenia, liver disease, or another specific clue emerges.

Her preferences determine the sequence whenever immediate safety allows meaningful choice.

Classify without forcing one cause#

PALM-COEIN organizes structural causes (polyp, adenomyosis, leiomyoma, malignancy or hyperplasia) and nonstructural causes (coagulopathy, ovulatory dysfunction, endometrial, iatrogenic, and not otherwise classified. Camila may occupy more than one category: leiomyoma, ovulatory dysfunction, and iatrogenic anticoagulant amplification can coexist.

Progressive results and interpretation#

Camila receives measured resuscitation and red-cell transfusion because of ongoing bleeding, orthostatic symptoms, low hemoglobin, and physiologic response; the decision is not based on a number alone. Gynecology and hematology review the last factor Xa inhibitor dose, kidney function, current bleeding, and thrombosis history. The anticoagulant is held during uncontrolled major bleeding. Reversal is considered under the major-bleeding pathway but is not automatic for every uterine bleed; current drug effect, hemodynamics, ongoing loss, and ability to achieve source control matter.

Acute uterine treatment is selected around her recent unprovoked VTE, hypertension, pregnancy result, fibroid anatomy, and preferences. High-dose estrogen-containing therapy is not chosen reflexively because of thrombotic history. A progestin-based strategy and procedural planning are coordinated. Tranexamic acid is discussed cautiously: thrombotic history, current anticoagulation, bleeding severity, and evidence limitations require specialist judgment rather than a universal prohibition or routine prescription. Exact drug and dose are intentionally not generalized.

Bleeding slows over the next day, hemodynamics normalize, and hemoglobin stabilizes after treatment. Hysteroscopy identifies a submucosal fibroid and a small endometrial polyp. Targeted tissue shows benign proliferative endometrium without hyperplasia or malignancy. The fibroid and polyp are treated through a uterus-preserving procedure after anticoagulant timing and anesthesia are coordinated.

Because the pulmonary embolism was unprovoked and recent, indefinite interruption would carry meaningful recurrence risk. Hematology and gynecology agree on resumption after hemostasis is secure, using the shortest clinically justified interruption and a product-specific plan. They also revisit whether the selected anticoagulant and schedule best balance uterine bleeding, recurrence prevention, kidney function, and access. Camila is not told to skip future doses whenever bleeding increases.

Iron replacement begins, with route chosen around severity, tolerance, absorption, speed of repletion, and preference. Menstrual suppression and structural follow-up are planned so iron is not poured into an ongoing uncontrolled loss.

Management plan#

Control current bleeding and support circulation#

Resuscitation, blood products, uterine medical therapy, procedural control, and anticoagulant management occur in parallel. Hemodynamic instability or failed medical control lowers the threshold for urgent operative or interventional treatment. Choice among hormonal therapy, antifibrinolytic therapy, intrauterine therapy, hysteroscopic treatment, embolization, ablation, and hysterectomy depends on stability, cause, contraindications, fertility, uterine preference, and available expertise.

Preserve thrombosis prevention intelligently#

The anticoagulant indication and event timing are verified. Minor manageable bleeding may be treated without interruption; major ongoing bleeding may require holding and sometimes reversal. Restart follows hemostasis and compares recurrent bleeding with thrombosis. A vague instruction to "ask the specialist" without a date and owner is unsafe.

Combined estrogen exposure, progestin options, intrauterine systems, and procedures are considered through current contraceptive and thrombotic eligibility. A cavity-distorting fibroid may make an intrauterine system less feasible until structure is addressed. Contraception and bleeding treatment overlap but are not identical decisions.

Treat iron deficiency and its cause#

Iron replacement has a target, monitoring interval, and tolerance plan. Hemoglobin improvement precedes full restoration of iron stores, so treatment does not stop at the first normal count. Persistent fatigue, pica, restless legs, or exercise limitation are reassessed. Other blood-loss sources are investigated if the gynecologic explanation is incomplete.

Create durable menstrual control#

After benign pathology and fibroid treatment, Camila chooses a progestin-based maintenance option compatible with her thrombotic history and uterine anatomy. The plan includes expected bleeding changes, failure criteria, contraception, fertility implications, and what to do if anticoagulation changes.

Escalation, referral, and safety net#

Emergency care is required for soaking protection hourly with lightheadedness, fainting, chest pain, severe breathlessness, confusion, cold or clammy skin, severe pelvic pain, shoulder pain, or possible pregnancy. A person with hemodynamic symptoms should not drive herself.

During treatment, persistent tachycardia, hypotension, rising lactate, ongoing rapid loss, falling hemoglobin, severe pain, fever, or failed medical control prompts urgent gynecologic and resuscitation escalation. New chest pain, breathlessness, unilateral leg symptoms, or neurologic change during anticoagulant interruption raises concern for recurrent thrombosis and requires immediate assessment.

Postprocedure fever, worsening pain, malodorous discharge, heavy recurrent bleeding, or faintness requires urgent review. Pathology results, anemia monitoring, anticoagulant restart, and menstrual treatment each have a named contact rather than one generic follow-up instruction.

Communication, shared decisions, and equity#

The clinician explains: "The anticoagulant makes bleeding easier, but it does not explain why your uterine bleeding became irregular over eight months. We are doing two things at once: controlling the blood loss safely and looking for the uterine or hormonal cause. We also need to protect you from another clot, so any interruption will be coordinated and as short as safely possible."

Camila's assumption that perimenopause means any bleeding is normal is corrected without blame. The team explains that cycle changes can occur, but persistent intermenstrual bleeding, anemia, and risk factors deserve evaluation. Cancer is discussed as one possibility that tissue can assess, not as a foregone conclusion.

Menstrual products, missed work, bathroom access, cultural beliefs, sexual history, fertility, and prior painful examinations are addressed privately. A professional interpreter is offered. Camila chooses who may participate. Trauma-informed examination includes explanation, permission at each step, the ability to pause, and alternatives when safe.

Medication cost and appointment rescheduling contributed to an unfinished anticoagulation-duration decision. The follow-up is scheduled before discharge, pharmacy supply is confirmed, and the plan states exactly when to resume the anticoagulant. Written instructions avoid euphemisms and use observable bleeding and emergency symptoms.

Follow-up and contingencies#

Early follow-up confirms bleeding trajectory, anticoagulant resumption, hemoglobin, iron tolerance, pathology, procedural recovery, and contraception. Later review checks iron stores, cycle pattern, blood pressure, VTE symptoms, and whether the maintenance plan is acceptable. A normal hemoglobin does not close persistent irregular bleeding.

If pathology shows hyperplasia or malignancy, gynecologic oncology and fertility counseling replace the benign pathway. If the cavity remains distorted or bleeding continues, alternative medical, hysteroscopic, radiologic, or surgical options are revisited. If a bleeding disorder history emerges, hematology expands evaluation rather than attributing every abnormal test to anticoagulation.

If future anticoagulant therapy changes, bleeding effects and interactions are reviewed prospectively. If anticoagulation is eventually stopped after a specialist risk discussion, menstrual disease still requires follow-up. If pregnancy becomes a goal, preconception review addresses anticoagulant safety, fibroid implications, anemia, hypertension, and fertility.

Reasoning traps and alternative pathways#

An unstable patient with uncontrolled hemorrhage may need immediate operative or interventional control. A positive pregnancy test redirects the case to early-pregnancy location and viability assessment. Postmenopausal bleeding has a different cancer-risk pathway. Lifelong heavy bleeding and procedural hemorrhage elevate inherited coagulopathy. A patient with active recurrent VTE may require a different anticoagulant and procedural balance than Camila.

Evidence limits and what could change#

Abnormal uterine bleeding research uses evolving symptom definitions and heterogeneous outcomes. PALM-COEIN improves organization but does not prove causation; multiple categories often coexist. Menstrual blood-loss estimates are imprecise, so quality of life, flooding, anemia, and function remain central.

Evidence for controlling heavy menstrual bleeding during anticoagulation is less complete than evidence in non-anticoagulated populations. Trials may exclude recent VTE, severe bleeding, cavity distortion, or complex comorbidity. Hormonal and antifibrinolytic choices therefore require individualized thrombotic and gynecologic assessment.

Anticoagulant interruption and restart evidence often comes from gastrointestinal or intracranial bleeding rather than uterine bleeding. Drug half-life, organ function, bleeding control, event timing, and thrombosis mechanism differ. New reversal agents, anticoagulants, and menstrual therapies may change options.

Sampling and imaging recommendations vary across systems, especially by age and endometrial-risk threshold. The durable approach is to stabilize, exclude pregnancy, classify causes, obtain tissue when risk warrants it, preserve thrombosis prevention, and verify resolution of both bleeding and iron deficiency.

Questions and answers

Is anticoagulation a complete explanation for heavy uterine bleeding?

No. Anticoagulation can amplify bleeding, but pregnancy, fibroids, polyps, adenomyosis, ovulatory dysfunction, endometrial disease, coagulopathy, and other causes still require evaluation.

What is the first question in acute heavy bleeding?

Determine hemodynamic stability and the severity and trajectory of blood loss. Fainting, chest pain, breathlessness, hypotension, impaired perfusion, or ongoing rapid bleeding can require emergency resuscitation and urgent gynecologic control.

Should an anticoagulant always be stopped when uterine bleeding occurs?

No. Interruption depends on bleeding severity, current drug effect, thrombosis indication and timing, source control, and the consequences of stopping. Major uncontrolled bleeding differs from manageable chronic heavy bleeding.

Is a normal pelvic ultrasound enough to exclude endometrial disease?

No. Ultrasound describes structure but cannot replace indicated endometrial assessment. Age, persistent irregular or intermenstrual bleeding, risk factors, and treatment failure determine whether sampling or hysteroscopy is needed.

Can estrogen-containing therapy be used to control bleeding in someone with prior venous thrombosis?

Estrogen can increase thrombotic risk, so the indication, timing of thrombosis, current anticoagulation, alternatives, and contraceptive eligibility must be reviewed with the treating clinicians. It is not a universal first choice.

What must follow control of the acute episode?

Treat iron deficiency, identify the bleeding mechanism, create a sustainable menstrual-suppression or procedural plan, clarify anticoagulant duration, monitor recurrence, and make sure pathology and imaging results reach a named clinician.
