Clear sight is not a complete measure of eye health. You can read the smallest line on a chart and still have an early problem in the retina, optic nerve, peripheral field, tear film, or eye pressure. Conversely, blurry vision may come from a correctable refractive error rather than a progressive disease. Protection begins by matching the examination and the response to your particular risk.
Age is only one variable. Symptoms, family history, and diabetes can all change what is appropriate. So can blood pressure, medicines, and prior surgery or injury. So can pregnancy, occupation, contact-lens use, and findings from earlier exams.
Understand what different checks can show#
A visual acuity test measures how clearly a person resolves standardized detail at a set distance. A refraction estimates the lens correction needed for nearsightedness, farsightedness, astigmatism, or age-related near-focus change. Neither alone rules out eye disease.
A comprehensive examination may include history, acuity, refraction, pupil responses, eye alignment and movement, peripheral-field testing, pressure measurement, microscopic examination of the front of the eye, and inspection of the retina and optic nerve through dilated pupils. Imaging or additional tests are added when findings or risk justify them.
Dilation briefly widens the pupil so more of the internal eye can be seen, and light sensitivity and blurred near vision can last for hours, so you may need to plan how you will get home. The National Eye Institute emphasizes that dilation helps detect conditions such as diabetic retinopathy, glaucoma, and age-related macular degeneration before you would necessarily notice them.
A screening is narrower. It applies a test or brief assessment to people without a known diagnosis to identify who might benefit from evaluation; screening evidence asks whether the full pathway, from invitation through diagnosis and treatment, improves meaningful outcomes with acceptable harms. It is not the same as examining someone with symptoms or known risk.
Eye health across the life course#
In infancy and childhood, visual development depends on both eyes delivering usable images to the brain, and amblyopia can develop when the brain favors one eye because of unequal focus, misalignment, or an obstruction. The USPSTF recommends vision screening at least once for children ages three to five to detect amblyopia or its risk factors. It found insufficient evidence to determine the balance for children younger than three; that does not mean concerns such as abnormal red reflex, persistent eye turning, or poor visual behavior should be ignored.
School-age children and young adults commonly develop refractive changes. Headaches, squinting, or moving close to screens can justify assessment. So can loss of place while reading or difficulty seeing the board. But those symptoms have multiple causes. Contact lenses require a prescription, fit assessment, and hygiene; cosmetic appearance does not make a lens risk-free.
In midlife, the natural lens becomes less flexible, making near focus harder. Presbyopia is common and not the same as cataract or glaucoma. Dryness may increase with age, environment, medicines, eyelid disease, or hormonal changes. New symptoms should not automatically be blamed on aging.
Later in life, cataract, macular degeneration, glaucoma, and systemic conditions become more common. The USPSTF's 2022 statement found insufficient evidence to determine the balance of primary-care screening for impaired visual acuity among asymptomatic adults 65 and older; this I statement is not a recommendation against eye care, and it does not apply to symptoms, diagnosed disease, or a comprehensive eye-care setting. It reflects a gap in direct evidence for a specific population-screening pathway.
Choose an exam interval by risk, not a slogan#
The NEI recommends discussing how often to have a dilated examination based on your own risk, and it highlights more frequent examination for people at higher risk of glaucoma and advises people with diabetes or hypertension to ask about an appropriate schedule. Prior findings may shorten an interval even when your vision feels unchanged.
Questions that help set timing include:
- Have vision, glare, color, contrast, depth, or peripheral awareness changed?
- Is there diabetes, hypertension, autoimmune disease, high myopia, or another relevant condition?
- Does glaucoma, retinal detachment, macular degeneration, or inherited eye disease run in the family?
- Has there been eye injury, surgery, radiation treatment, or long-term use of a medicine affecting the eye?
- Are contact lenses used, and have pain, redness, discharge, or light sensitivity occurred?
- Does work or sport create chemical, projectile, impact, or radiation hazards?
An eye professional can choose the examination elements and interval from your answers. A routine appointment should not substitute for urgent care when symptoms change abruptly.
Protect eyes from injury and ultraviolet radiation#
Ordinary prescription glasses are not a substitute for task-rated protection. Use safety eyewear appropriate to the hazard for power tools, chemicals, and yard work. Use it for construction, laboratory tasks, and impact sports. Workplace standards may specify the required type and fit. Eye protection should work with your prescription correction, not be displaced by it.
For chemical contact, begin immediate irrigation and obtain poison-control or emergency guidance; do not delay flushing while searching for a specific clinic. Penetrating or high-velocity injuries need urgent assessment. Do not press on an injured eye or attempt to remove an embedded object.
NEI advises sunglasses that block 99% to 100% of UVA and UVB radiation. A brimmed hat adds shade. Dark lenses without adequate UV protection are not equivalent. UV protection addresses cumulative radiation and does not make staring at the sun safe; eclipses and high-intensity light require specific certified protection.
Contact-lens habits are infection prevention#
CDC describes contact lenses as medical devices. Wash and thoroughly dry your hands before handling them. Follow your prescribed wear and replacement schedule. Use the recommended disinfecting system, not water or saliva, and do not “top off” old solution. Keep lenses away from shower, pool, and hot-tub water. Sleep in lenses only if your eye-care professional and the lens instructions specifically permit it.
Redness, worsening pain, light sensitivity, discharge, or sudden blur in a lens wearer may signal corneal inflammation or infection. Remove the lens and contact an eye-care professional promptly; do not resume wear until advised. Keep backup glasses available.
General health and eye health meet#
Diabetes can affect retinal blood vessels and raise the risk of several eye conditions; blood-pressure disorders can be reflected in retinal vessels, while smoking increases the risk of cataract and age-related macular degeneration. Managing systemic conditions supports eye health but does not replace an eye examination.
Review your medicines and supplements with both your prescriber and your eye clinician. Some medicines can affect dryness, pupil function, eye pressure, the lens, or the retina. The relevance depends on dose, duration, susceptibility, and examination findings; never stop a prescribed medicine solely because it appears on a general list.
Sleep, activity, and a varied dietary pattern support general health. Be cautious with supplements marketed as universal eye protection. Benefit may be limited to specific formulations and diagnosed risk groups; high doses can cause harm or interactions. Screens cause strain more often than injury#
Concentrated near work tends to reduce blinking and sustain focusing effort. Dryness, burning, blur that clears with blinking, headache, and difficulty shifting focus can follow. Adjust distance, text size, and glare. Adjust lighting, posture, and break patterns. Blink fully and take periodic distance-viewing breaks. The common “20-20-20” reminder is a practical habit, not a precisely validated dose.
Persistent pain, double vision, or one-sided symptoms should not be assigned to that label without assessment. Neither should marked redness, light sensitivity, or declining vision be assigned to “screen fatigue” without assessment. Children with new visual behavior or school difficulty also deserve a broader evaluation rather than an assumption about devices alone.
Know the urgent pattern#
NEI identifies a sudden increase in floaters, flashes, or a curtain or shadow in the field as possible retinal-detachment symptoms requiring immediate assessment. Sudden loss of vision, severe eye pain, a painful red eye with nausea or blur, new double vision with neurologic symptoms, or significant trauma also warrants urgent care.
For gradual change, schedule an eye examination rather than waiting until function is severely limited. Note whether the change affects one or both eyes, near or distance, central or peripheral vision, and whether it is constant or intermittent. Covering one eye at a time can reveal asymmetry, but it is not a diagnostic test.

Is a vision screening the same as a comprehensive eye exam?
No. A screening usually checks a limited function or risk and identifies who may need evaluation. A comprehensive exam can assess refraction, eye structures, pressure, visual fields, and other findings based on the person.
How often should adults get a dilated eye exam?

Can a person have eye disease while seeing clearly?
