Painful Red Eye With Photophobia and Reduced Vision

Pain, light sensitivity, and a real drop in acuity move a red eye out of the conjunctivitis pathway. Protect sight first, then work out which examination finding decides the next step.

Learning objectives#

Initial presentation#

A 42-year-old restaurant manager arrives at an urgent-care clinic with pain and redness in the left eye. The eye felt gritty when she removed a soft contact lens the previous evening. By morning, light from her phone was painful and her vision through that eye seemed foggy. The pain has intensified despite keeping the lens out. She noticed watering but little thick discharge. She has no fever, new rash, weakness, or pain in the other eye.

She usually wears monthly replacement lenses for long work shifts. Two nights ago she fell asleep in them. She sometimes showers while wearing them and recently extended the same pair beyond the intended replacement interval to save money. She uses commercial disinfecting solution and denies rinsing the lenses with tap water. She has not used prescription eye drops, although a coworker offered an old bottle that she wisely did not accept.

There was no high-speed work, grinding, or chemical splash. There was no blunt injury, eye surgery, or injection. She has no severe headache, vomiting, or colored halos around lights. There is no new joint swelling, mouth ulcer, or inflammatory bowel symptoms. There are no breathing symptoms or known autoimmune disease. She takes no immunosuppressive medicine. She recalls one mild episode of viral conjunctivitis years ago, but says this pain is different.

Her temperature is normal, blood pressure is 128/76 mm Hg, and pulse is 86 per minute. She is uncomfortable under the examination light but is not systemically ill. The eyelids are not markedly swollen. There is diffuse left-eye redness with more injection near the cornea. Extraocular movements are full and do not cause deep orbital pain. There is no proptosis. The clinic has a visual chart, cobalt-blue illumination, fluorescein strips, a slit lamp, and a calibrated tonometer. An ophthalmologist can accept an urgent referral if the handoff establishes need.

Problem representation#

This is an immunocompetent adult contact-lens wearer with an acute unilateral painful red eye, marked photophobia, watering, and objectively reduced vision after overnight lens wear. She has no clear trauma, chemical injury, orbital signs, or systemic illness. That cluster makes routine viral, allergic, or uncomplicated bacterial conjunctivitis an unsafe organizing diagnosis.

The immediate problem is a potentially vision-threatening corneal or intraocular process that needs focused examination and same-day ophthalmic assessment. Contact-lens use raises microbial keratitis. Severe pain, photophobia, and reduced acuity also require active consideration of acute angle closure, anterior uveitis, and scleritis. The decision to escalate does not depend on naming the organism or completing every test at urgent care.

Three details control the initial pathway. First, acuity must be measured in each eye rather than described only as blurry. Second, the cornea and anterior chamber need direct assessment. Third, tonometry is useful only after trauma and open-globe concern have been addressed; pressure is one discriminator, not a universal safety test.

Prioritized differential#

1. Microbial keratitis#

Reasoning for: Contact-lens wear, overnight use, and escalating pain after lens removal form a high-risk cluster. So do photophobia, tearing, ciliary-pattern injection, and reduced acuity. The CDC identifies worsening eye pain, light sensitivity, sudden blurred vision, and abnormal tearing or discharge as warning symptoms in a contact-lens wearer. A focal corneal infiltrate or epithelial defect would move this diagnosis to the front.

Reasoning against or still uncertain: No corneal finding has yet been documented, and several infectious causes can look similar early. Bacterial infection is common, but viral, fungal, and amebic disease cannot be separated by symptom labels alone. The time course is shorter than many Acanthamoeba presentations, yet water contact and poor response to initial therapy would increase that concern.

2. Acute angle-closure crisis#

Why it cannot be missed: A sudden pressure rise can threaten sight and requires expeditious ophthalmic management. Severe ocular pain, blurred vision or halos, and headache would support this branch. So would nausea or vomiting, corneal haze, a shallow chamber, and a poorly reactive mid-dilated pupil.

Case evidence: She lacks systemic symptoms and halo history, and contact-lens risk points elsewhere. Those are probability modifiers, not exclusions. Pupil shape, corneal clarity, anterior-chamber depth, and intraocular pressure when safe are still required. A high pressure with the characteristic examination would reorganize management immediately.

3. Acute anterior uveitis#

Reasoning for: Uveitis can begin suddenly with redness, pain, photophobia, blurred vision, and sometimes floaters. Consensual photophobia, ciliary flush, anterior-chamber cells and flare, or a small irregular pupil would strengthen this branch.

Reasoning against or still uncertain: The contact-lens history and possible corneal process are more prominent. She reports no prior uveitis or suggestive systemic history, but many cases have no known systemic diagnosis at first presentation. Absence of that history does not eliminate intraocular inflammation.

4. Scleritis#

Reasoning for: Scleritis can produce severe, deep pain and globe tenderness. It can produce pain radiating to the face, pain with eye movement, and a violaceous deep-red hue. It may threaten vision and can be linked to systemic inflammatory or infectious disease.

Reasoning against or still uncertain: Her pain feels anterior and gritty rather than deep or boring, movements are painless, and no globe tenderness or violaceous hue has been recorded. A proper examination is still needed because superficial redness can obscure deeper inflammation and corneal or uveal involvement can coexist.

5. Corneal abrasion or retained foreign body#

A surface injury can cause pain, tearing, photophobia, blepharospasm, and fluorescein uptake. There is no clear trauma, but a tiny foreign body or injury during lens removal remains possible. An abrasion without stromal infiltrate follows a different pathway, although a contact-lens-related epithelial defect carries infection risk and still warrants careful follow-up.

6. Conjunctivitis, episcleritis, or dry-eye disease#

These common disorders can cause redness, irritation, tearing, itching, or discharge. Pronounced pain, direct photophobia, a meaningful acuity loss, corneal opacity, or an abnormal pupil argues against managing the case as an uncomplicated surface irritation. A benign condition may coexist, but it should not be used to explain away warning features.

7. Other cannot-miss alternatives#

An open globe or intraocular foreign body becomes central after penetrating or high-velocity injury, a peaked pupil, very low pressure, uveal tissue, or a positive leak test. Chemical injury requires immediate irrigation before a prolonged history. Endophthalmitis rises after recent intraocular surgery, injection, penetrating injury, or bloodstream infection. The risk is particularly high with pain, rapidly falling vision, hypopyon, or poor red reflex. Orbital cellulitis requires urgent imaging and specialty care when fever, proptosis, impaired movements, pain with movement, or optic-nerve dysfunction appears. Acute retinal or optic-nerve disease can reduce vision, although prominent anterior pain and redness point toward another structure.

Focused history and examination#

The history is organized by decisions rather than by a generic eye checklist.

Visual acuity is checked early with usual correction, one eye at a time, and pinhole when feasible. The right eye reads 20/20. The left reads 20/100 and improves only to 20/80 with pinhole. This confirms a clinically meaningful monocular deficit rather than blur caused only by uncorrected refractive error.

The clinician then checks pupils for shape, reactivity, asymmetry, and a relative afferent defect; confrontational fields; color comparison when possible; eye movements; lids and lashes; preauricular nodes; patterns of injection; corneal clarity; anterior-chamber appearance; and red reflex. Direct and consensual light responses are noted without treating either as a stand-alone diagnosis.

The left pupil is round and reactive and is not fixed or mid-dilated. There is no relative afferent pupillary defect. A small pale opacity is visible just below the visual axis. Because no open-globe mechanism or sign is present, a gentle slit-lamp and fluorescein examination is appropriate. Manipulation stops if integrity becomes uncertain.

Diagnostic strategy#

Tests that change the immediate decision#

Slit-lamp examination shows a focal epithelial defect overlying a denser stromal infiltrate, with surrounding corneal haze. There is mild inflammatory material in the anterior chamber but no hypopyon. The fluorescein pattern is focal rather than a branching dendritic pattern. No retained foreign body is found after careful lid assessment. These findings make infectious keratitis the organizing diagnosis; mild anterior-chamber inflammation can be secondary and does not automatically mean primary uveitis.

Once the cornea is assessed and open-globe concern remains low, intraocular pressure is measured in both eyes. It is within the local reference range and similar between eyes. The anterior chamber is not shallow and the pupil remains reactive. That combination lowers the likelihood of an acute angle-closure crisis at this moment. It does not declare the eye safe, because serious keratitis and uveitis can occur with normal pressure.

The ophthalmologist, not the urgent-care clinic, makes that decision. The options are corneal scraping, smear, culture, polymerase-chain-reaction testing, or confocal imaging. The 2024 American Academy of Ophthalmology keratitis guideline supports selective microbiologic testing when an infiltrate is central, large, deep, atypical, chronic, recurrent, linked to prior corneal surgery, or failing initial therapy. Acanthamoeba testing becomes more important with water contact, pain that seems disproportionate, atypical features, or poor response.

Tests that are not routine here#

A broad blood panel does not diagnose a focal corneal infection and would delay the eye handoff. Autoimmune serologies are not a first response to every red eye; they become targeted after ophthalmic confirmation of scleritis or uveitis and a history or examination that identifies a plausible systemic pathway. Orbital computed tomography is unnecessary without proptosis, movement restriction, orbital trauma, or suspected foreign body. Brain imaging does not replace an ocular examination.

Culturing the contact lens or case may occasionally support an investigation, but it does not prove that the same organism is invading the cornea. A superficial conjunctival swab is not a substitute for a specialist corneal specimen. A response to a topical anesthetic can localize some pain to the surface, but it does not distinguish harmless abrasion from infectious keratitis and must not be used as permission to discharge.

Progressive results and interpretation#

The urgent-care clinician calls the ophthalmologist with acuity, lens history, and pupil findings. The call reports slit-lamp and fluorescein findings, pressure, and absence of trauma or orbital signs. The ophthalmologist accepts same-day evaluation and asks that the patient bring the lens, case, solution, and all eye drops without placing anything back in the eye.

At the eye service, repeat acuity is unchanged. A cornea specialist confirms a paracentral microbial keratitis with an epithelial defect and stromal infiltrate close enough to the visual axis to threaten vision. The size, location, and anterior-chamber reaction meet the service's criteria for obtaining a corneal specimen before treatment, without delaying empiric therapy. Gonioscopic and anterior-segment findings do not support angle closure. The deep scleral vessels and globe are not characteristically tender, making scleritis unlikely. The inflammatory cells are interpreted as a reaction to corneal infection rather than isolated anterior uveitis.

Initial microscopy is nondiagnostic. Treatment therefore remains empiric and locally protocolized rather than falsely organism-specific. The next day's culture grows a bacterium consistent with the clinical setting, and susceptibility information supports narrowing the regimen under the cornea specialist. The case intentionally omits drug concentrations and schedules because intensive ocular therapy requires direct examination, preparation-specific instructions, and close review.

At the first follow-up, pain is improved and the epithelial defect is smaller, but the infiltrate has not disappeared. That is a favorable trajectory, not proof of cure. If pain, infiltrate, or vision had worsened, the specialist would reconsider resistance and adherence. The review would cover fungal or amebic infection, herpetic disease, toxic drop effects, and an incorrect noninfectious diagnosis.

Management plan#

Immediate actions at the first site#

The lens remains out and is not reinserted. The patient is advised not to patch the eye, share drops, or drive herself with impaired monocular vision. The clinic avoids dispensing topical anesthetic for home use and avoids redness-relief or corticosteroid drops. An ophthalmologist may later use a corticosteroid in selected bacterial keratitis after effective antimicrobial therapy and evidence of control, but unsupervised early steroid use can worsen or disguise infection, especially when the organism is uncertain.

Pain is managed with nonocular systemic options. The choice follows a check of allergies, pregnancy, and kidney disease. It also follows a check of liver disease, ulcer or bleeding risk, and other medicines. Comfort treatment must not delay the corneal assessment. The patient travels by a safe arrangement to the eye service; emergency transport would be appropriate if vision were rapidly deteriorating, the patient were systemically ill, or no safe private transport were available.

Ophthalmology-directed treatment#

The cornea service starts an intensive topical antimicrobial regimen selected for the severity, local resistance patterns, lens-associated organisms, specimen plan, allergies, pregnancy status, ocular surface, and ability to follow the schedule. Exact dosing is deliberately omitted. The regimen is narrowed or changed only with the clinical course and microbiology, not by culture in isolation.

Follow-up is early because worsening can be rapid and adherence to a demanding schedule may be difficult. The plan confirms that the patient can obtain the medicine immediately, understands administration, has help if vision limits self-care, and can return after hours. The old lenses, storage case, and potentially contaminated solution are not reused. Future lens wear resumes only after the eye specialist confirms healing and reviews safe replacement and hygiene.

Alternative branches and contraindications#

Escalation, referral, and safety net#

Painful red eye plus photophobia and reduced acuity already meets a same-day ophthalmic threshold. Immediate emergency-department or eye-casualty transfer is warranted when there is rapidly falling vision, a fixed or irregular pupil, very high or unexpectedly low pressure, corneal whitening or thinning, hypopyon, suspected open globe, chemical injury, severe headache with vomiting and ocular signs, recent intraocular procedure with pain and vision loss, proptosis, restricted movements, fever with orbital findings, or inability to obtain urgent specialist review.

While awaiting review, new loss of part of the visual field, a curtain or many new floaters, inability to perceive light normally, increasing corneal opacity, severe vomiting, new weakness, or escalating pain prompts emergency services rather than a delayed office message. The patient should not drive when vision is impaired or after medicines that affect alertness.

The safety net names a destination and time, not merely "follow up with eye care." It includes the eye service's after-hours route, what to bring, what not to place in the eye, and what to do if transportation fails. A failed handoff is treated as unresolved emergency risk rather than as patient noncompliance.

Communication, shared decisions, and equity#

The clinician explains the reasoning without announcing a final organism: "The surface of your cornea has an abnormal area, and your vision is measurably lower. Contact lenses can raise the chance of a serious corneal infection. Other inflammatory and pressure problems can look similar, so you need an eye specialist today."

The patient worries that admitting extended lens use will lead to blame. The team responds nonjudgmentally because accurate details improve care. Cost drove her to stretch the replacement interval, so prevention cannot end with an instruction to do better. The plan explores affordable glasses, insurance or assistance options, access to replacement lenses and solution, and whether work rules make lens-free shifts difficult.

Professional interpretation and translated instructions are arranged if preferred. Materials account for low vision, color-vision differences, and reading level. Teach-back asks the patient to describe why the visit is urgent, where she is going, which drops to avoid, and which changes require emergency action.

Shared decision-making applies to feasible transport, pain-control options, future lens choices, and how to overcome treatment barriers. It does not make delayed specialist assessment equivalent to same-day care when vision is threatened. The team also avoids race-based shortcuts: ancestry can affect angle-closure probability at a population level, but the patient's anatomy and examination determine this case.

Follow-up and contingencies#

The cornea service reassesses acuity, epithelial-defect size, and infiltrate dimensions and depth. It reassesses anterior-chamber response, pain, and adherence. It reassesses toxicity and culture information at intervals set by severity. Each comparison uses the same documented landmarks where possible. Improvement requires a coherent trend; less pain alone is insufficient if the infiltrate expands or acuity falls.

If the course stalls, the team repeats the history about water, soil or plant matter, and lenses. It asks again about prior drops, immune status, and herpes-family symptoms. It verifies technique and access before labeling treatment failure. Repeat scraping, expanded cultures, molecular tests, or confocal microscopy may be considered. That applies when amebic, fungal, mycobacterial, viral, or resistant disease becomes more plausible.

After healing, the clinician confirms whether a scar affects the visual axis, whether corrective lenses need updating, and when lens use may safely resume. Prevention includes hand hygiene, proper disinfection, no topping off old solution, keeping lenses away from water, avoiding unapproved overnight wear, replacing lenses and cases on schedule, and carrying usable glasses.

The primary-care clinician follows any systemic branch raised by ophthalmology. Confirmed recurrent uveitis or scleritis may justify targeted evaluation for inflammatory or infectious disease. A single corneal infection without supporting features does not justify an indiscriminate autoimmune panel.

Reasoning traps and alternative pathways#

An alternate version with a fixed mid-dilated pupil, shallow chamber, and hazy cornea would become an acute angle-closure emergency. High pressure, headache, and vomiting would complete that picture. Cells and flare without an epithelial defect or infiltrate could move primary anterior uveitis upward. Deep boring pain, globe tenderness, and violaceous injection would favor scleritis. Copious discharge with preserved acuity and little pain might follow a conjunctivitis pathway, but only after warning findings are absent.

Evidence limits and what could change#

Red-eye evidence spans specialty guidelines, observational studies, microbiology, and service-triage guidance. Examination quality and equipment differ across primary care, urgent care, emergency departments, optometry, and ophthalmology. Fluorescein and pressure readings are operator-dependent, and not every corneal infection has a classic appearance.

The 2024 bacterial-keratitis guideline supports urgent organism-aware treatment while reserving cultures for selected presentations; local resistance and access affect implementation. CDC guidance emphasizes immediate eye review for contact-lens wearers with pain, light sensitivity, or sudden blur and highlights the diagnostic challenge of Acanthamoeba. The angle-closure guideline identifies an acute crisis as an ophthalmic emergency, while the Academy's uveitis material confirms that pain, light sensitivity, and blur can overlap.

This culture result described in this analysis does not mean contact-lens keratitis is always bacterial, and early improvement does not guarantee full visual recovery. A different corneal pattern, water history, or immune state could change the leading diagnosis and treatment. So could a different pressure, pupil, trauma mechanism, surgery history, or treatment response.

Questions and answers

Is every red eye with discharge conjunctivitis?

No. Pain, photophobia, reduced acuity, corneal opacity, an abnormal pupil, trauma, contact-lens use, or a deep tender eye requires another pathway and often urgent ophthalmic assessment.

Can normal eye pressure exclude an eye emergency?

No. It makes an acute pressure crisis less likely at that moment, but keratitis, uveitis, scleritis, open-globe injury, endophthalmitis, and other threats can occur without high pressure.

Should a painful contact-lens eye be treated with leftover drops?

No. The lens should be removed, reinsertion avoided, and urgent eye assessment arranged. Unsupervised anesthetic or steroid drops can cause harm or obscure infection.

Does pain out of proportion prove Acanthamoeba keratitis?

No. It is a clue, not a diagnosis. Water contact, lens practices, examination, response over time, and specialist testing all matter.
