The Hot Swollen Knee and the Aspiration Decision

A crystal seen in joint fluid can explain inflammation without excluding infection. The safe pathway integrates aspiration, microbiology, drainage, treatment constraints, and follow-up.

Learning objectives#

Initial presentation#

A 64-year-old man presents to an urgent care center because his right knee became painful and swollen over eighteen hours. He awoke with stiffness, then developed rapidly increasing pain and could no longer bear weight by afternoon. He felt chilled during the drive but did not measure a temperature at home. There was no fall, twist, or direct blow.

His history includes hypertension, stage 3 chronic kidney disease, and one episode of first-toe pain four years earlier that was called gout without aspiration. He takes a thiazide-like diuretic and a blood-pressure medicine. He does not take an anticoagulant, immunosuppressive medicine, or urate-lowering therapy. He has no joint replacement. Three days ago he scraped the front of his right shin while repairing a fence. He washed the shallow abrasion, which now has a dry crust without drainage.

He reports no recent joint injection or surgery, known bloodstream infection, or intravenous drug use. There is no diarrhea, urethral symptoms, or eye symptoms. There is no new rash or pain in another joint. He has not had a known tick bite and has not recently spent time in a region where Lyme disease is common. A private sexual history has not yet been obtained. He has no known inflammatory arthritis.

At urgent care the temperature is 38.0 degrees Celsius, blood pressure 132/76 mm Hg, pulse 102 per minute, respiratory rate 18 per minute, and oxygen saturation normal. He appears uncomfortable but alert. The right knee has a large effusion, warmth, and diffuse tenderness. Active and passive flexion are markedly limited by deep joint pain. There is no fluctuance over the prepatellar bursa. The shin abrasion is several centimeters below the joint and has no spreading erythema. Distal pulses, sensation, and foot movement are intact. No other joint is swollen.

The center can obtain a plain radiograph but cannot perform or process an urgent sterile joint aspiration. The clinician arranges emergency-department transfer rather than treating presumed gout and asking him to return if it persists.

Problem representation#

This is an older adult with acute atraumatic monoarthritis of a native knee, fever, mild tachycardia, inability to bear weight, a large effusion, severe pain with passive motion, chronic kidney disease, a remote unconfirmed crystal-like episode, and a recent skin break on the same limb. There is no shock, prosthesis, obvious cellulitis, or established diagnosis.

The central problem is septic arthritis versus crystal arthritis, with possible coexistence and no bedside feature that safely separates them. Rapid aspiration is needed because infection can damage cartilage and become systemic, while premature antibiotics in a stable patient can reduce culture yield. At the same time, the team must not let the desire for a perfect specimen delay treatment if sepsis develops.

Prioritized differential#

1. Septic arthritis of the native knee#

Reasoning for: Acute severe monoarticular pain, effusion, fever, tachycardia, and pain with passive movement fit an intra-articular infection. Age, chronic kidney disease, and a recent skin break may increase risk. Septic arthritis can occur without dramatic fever, leukocytosis, or an obvious portal.

Why it cannot be missed: Delayed drainage and antimicrobial treatment can cause bacteremia, sepsis, and cartilage injury. They can cause osteomyelitis, chronic pain, and loss of function. Clinical findings and serum tests cannot confirm or exclude it, so microbiologic sampling and orthopedic assessment are central.

2. Crystal arthritis, including calcium pyrophosphate disease or gout#

Reasoning for: Abrupt intense inflammation of a knee in an older adult is compatible with calcium pyrophosphate crystal arthritis. The remote first-toe episode and diuretic use make gout plausible. Fever, leukocytosis, and high inflammatory markers can occur in either crystal disease or infection.

Reasoning against or still uncertain: The earlier toe diagnosis was not crystal-proven. Serum urate, even if elevated, would not establish that the knee is gout. Most importantly, identifying crystals would not exclude a simultaneous bacterial infection.

3. Hemarthrosis or occult structural injury#

An unrecognized twist, osteochondral injury, fracture, bleeding disorder, or anticoagulant effect can produce an acute effusion. There is no trauma history or anticoagulant use, but platelet count, coagulation context, radiography, and aspirate appearance refine this branch. Bloody fluid can also result from a traumatic tap and should not end the infection evaluation.

4. Inflammatory or reactive arthritis#

Rheumatoid arthritis, psoriatic arthritis, spondyloarthritis, or reactive arthritis can begin asymmetrically. Other-joint symptoms, prolonged morning stiffness, and psoriasis would support this group. So would inflammatory bowel disease, preceding gastrointestinal or genitourinary illness, eye inflammation, and enthesitis. An acute single hot joint still requires infection assessment before immunosuppression.

5. Periarticular infection or inflammation#

Cellulitis, prepatellar bursitis, abscess, osteomyelitis, and necrotizing soft-tissue infection can resemble joint disease or coexist with it. The large effusion and severe passive-motion pain point toward the joint. Rapidly spreading erythema, crepitus, pain beyond visible findings, skin necrosis, or systemic toxicity would redirect the emergency pathway and surgical urgency.

6. Epidemiology-dependent infection#

Disseminated gonococcal infection can cause tenosynovitis, skin lesions, migratory pain, or purulent arthritis, sometimes without genital symptoms. Lyme arthritis more often produces a large-joint effusion in an epidemiologically compatible setting and requires the recommended serum antibody strategy rather than indiscriminate synovial culture. Tuberculous or fungal arthritis is usually more indolent but becomes relevant with compatible geography, immune status, or chronicity. These questions are asked confidentially and proportionately rather than attached to assumptions about identity.

Focused history and examination#

The focused history tests branches that would change sampling, treatment, or destination:

The examination starts with repeated vital signs, mental state, perfusion, and a sepsis screen. Both legs and all major joints are inspected. The knee examination documents effusion, warmth, and erythema. It documents focal bursal swelling, active and passive range, ligament stability only as tolerated, and distal neurovascular status. Skin is examined for wounds, cellulitis, and pustules. It is examined for petechiae, tophi, psoriasis, and pressure injury. Tendons are checked for tenosynovitis. Heart, spine, and neurologic findings are assessed when bacteremia or endocarditis is plausible. The same applies when spinal infection or another focus is plausible.

The clinician does not repeatedly force motion to prove pain. Absence of fever, redness, or leukocytosis would lower some probabilities but would not clear the joint.

Diagnostic strategy#

Aspirate the joint promptly and correctly#

In the emergency department, orthopedics and the emergency clinician agree that the patient is stable enough to obtain samples before empirical antimicrobials. Arthrocentesis uses strict sterile technique and a route that avoids the shin abrasion and any overlying inflamed skin. If landmarks were uncertain or the first attempt yielded no fluid, ultrasound guidance could confirm an effusion and intra-articular needle position. Saline injection to manufacture a specimen would not be used.

Synovial fluid is prioritized for bacterial culture, white-cell count with differential, crystal analysis under polarized light, and Gram stain. When volume is limited, bacterial identification takes priority. Additional fluid may be inoculated into blood-culture bottles according to the laboratory's procedure, but a properly labeled sterile-tube specimen is still sent. The team records whether antibiotics were given before collection.

Two sets of blood cultures are collected because the patient is febrile and bacteremia is plausible. Urogenital and extragenital nucleic-acid or culture specimens would be added if the confidential history or examination supported disseminated gonococcal infection. Lyme serology would be appropriate only with a compatible epidemiologic and clinical pattern. Neither test is ordered as part of a generic monoarthritis panel.

Use serum tests and imaging for defined roles#

Complete blood count, metabolic measures, C-reactive protein, and erythrocyte sedimentation rate help describe inflammation. They also identify organ constraints and provide a baseline for response. None rules septic arthritis in or out. Serum urate does not determine the cause of the acute knee.

Plain radiographs look for fracture, advanced osteoarthritis, and chondrocalcinosis. They look for destructive change, foreign material, and a baseline for later comparison. Ultrasound can confirm effusion and guide aspiration but cannot prove sterility. Magnetic resonance imaging is reserved for suspected osteomyelitis, deep abscess, difficult anatomy, or failure to improve; obtaining it should not delay aspiration and orthopedic care in a straightforward large-joint emergency.

Synovial white-cell count is a probability modifier. A count above 50,000 cells per microliter is suggestive according to the SANJO guideline, but it is not diagnostic. A count below that level cannot exclude infection, especially after antibiotics or in immune compromise. The percentage of neutrophils, gross appearance, Gram stain, and crystals are also contextual. Culture remains central, and a negative Gram stain has limited sensitivity.

Progressive results and interpretation#

Radiography shows a large effusion, mild osteoarthritis, and faint chondrocalcinosis without fracture or destructive bone change. Blood leukocyte count is 12.4 x 10^9/L and C-reactive protein is elevated. Kidney function is close to the patient's usual stage 3 range. Serum urate is within the laboratory reference interval. These results support inflammation and identify medication constraints but do not settle the diagnosis.

Arthrocentesis produces 48 mL of cloudy yellow fluid. Synovial white-cell count is 46,000 cells per microliter with 91 percent neutrophils. Polarized microscopy identifies positively birefringent rhomboid calcium pyrophosphate crystals. Gram stain shows many white cells but no organism.

The crystals, chondrocalcinosis, and age make an acute calcium pyrophosphate flare highly plausible. They do not explain away the fever, skin break, inability to bear weight, or infection risk. The count below 50,000 and the negative Gram stain also fail to exclude septic arthritis. The team records the interim conclusion as crystal-proven arthritis with bacterial coinfection still unresolved, not simply pseudogout.

After aspiration and blood cultures, empirical intravenous antibacterial therapy is started through the local native-joint infection pathway. Exact agents and doses are intentionally omitted because selection depends on local resistance, Gram-stain findings, allergy details, kidney function, prior organisms, and the likelihood of gonococcal, gram-negative, or resistant gram-positive infection. The patient is admitted, and orthopedics, infectious disease, and rheumatology coordinate care.

The next morning the synovial culture grows Staphylococcus aureus. Susceptibility testing later supports narrowing to targeted therapy. Blood cultures remain negative. The result establishes bacterial septic arthritis coexisting with calcium pyrophosphate crystals. The shin abrasion is a possible portal but cannot be proven as the source.

Arthroscopic lavage yields additional purulent fluid and tissue for culture. No adjacent osteomyelitis is found. Fever and pain improve after drainage and targeted therapy, C-reactive protein begins to fall, and supervised motion is introduced after infection control and drain removal. The initial crystal finding remains true, but it was not the whole diagnosis.

Management plan#

Sequence sampling, treatment, and drainage#

If the patient is hemodynamically stable, prompt aspiration before empirical antibacterial therapy improves the chance of identifying the pathogen. If sepsis, shock, or another rapidly deteriorating state is present, resuscitation and antimicrobials cannot wait for a delayed procedure. Samples are obtained as quickly as feasible without turning diagnostic perfection into unsafe inaction.

After sampling, empirical therapy covers the likely organisms through the hospital's current pathway and is adjusted for local resistance, immune status, allergy, kidney and liver function, and the suspected route. Results from Gram stain, blood and synovial cultures, susceptibility testing, and source assessment guide narrowing. Drug choice, route, monitoring, and duration require infectious disease, pharmacy, and orthopedic review. No operational dosing schedule is supplied here.

Large-joint septic arthritis requires a drainage strategy as well as antimicrobials. Options include repeated needle aspiration, arthroscopic lavage, or open surgery. Joint anatomy, clinical stage, and adhesions influence the choice. So do cartilage or bone damage, pathogen, and response. So do anesthesia risk and local expertise. In this case, purulent knee infection and functional threat favor arthroscopic lavage. Failure of pain, fever, inflammatory markers, drainage, or function to improve triggers reassessment for inadequate source control and possible repeat surgery.

Treat crystal inflammation without masking infection#

Pain relief, rest in the earliest phase, ice when comfortable, and an individualized anti-inflammatory plan can support recovery. Nonsteroidal anti-inflammatory medicines may be unsafe with chronic kidney disease, gastrointestinal bleeding, anticoagulation, heart failure, or some drug combinations. Colchicine requires attention to kidney and liver function, timing, toxicity, and interactions. Systemic glucocorticoids can worsen or obscure uncontrolled infection. Intra-articular glucocorticoid injection is contraindicated while septic arthritis remains possible and is not performed in this case.

Once infection is controlled, rheumatology helps decide whether additional crystal-directed treatment is needed. One calcium pyrophosphate flare does not automatically justify long-term urate-lowering therapy. Metabolic contributors to calcium pyrophosphate disease are evaluated when age, recurrence, or the broader clinical pattern supports testing.

Analgesia is adjusted for kidney function, sedation risk, falls, and interactions. Prolonged immobilization can cause stiffness, weakness, thrombosis, and functional loss, so rehabilitation begins as soon as the surgical and infection teams judge motion safe.

Escalation, referral, and safety net#

Emergency escalation is required for hypotension, confusion, rigors, breathing difficulty, rapidly spreading redness, severe pain beyond the joint findings, skin necrosis, new neurologic or vascular deficit, uncontrolled bleeding, rapidly increasing swelling, or another sign of sepsis or limb threat. A person with a hot swollen joint and systemic illness should not wait for an office aspiration or a routine imaging appointment.

Orthopedic involvement is early for a large native joint because diagnostic aspiration, drainage, and joint preservation are time-sensitive. Infectious disease review guides empirical and targeted therapy, culture-negative reasoning, resistant organisms, bacteremia, and treatment monitoring. Rheumatology helps interpret crystals, inflammatory mimics, and longer-term prevention after the infection branch is secure. Emergency or hospital medicine coordinates organ assessment and sepsis care.

At discharge, return immediately for recurrent fever, chills, increasing pain or swelling, drainage, spreading redness, new weakness or numbness, inability to bear weight after initial improvement, medicine reaction, severe diarrhea, or loss of function. Instructions name the responsible service and emergency destination rather than relying on an unanswered general inbox.

Communication, shared decisions, and equity#

The clinician explains the two-process possibility in plain language: "The fluid contains crystals that can cause a very painful flare. Crystals do not guarantee that the joint is free of bacteria, and your other findings keep infection on the list. We sent cultures and are treating that risk while the laboratory finishes its work."

The discussion avoids implying that gout or calcium pyrophosphate disease is a punishment for diet or personal choices. Medicines, kidney function, and genetics shape crystal disease. So do age, metabolic conditions, and many other factors. Nutrition counseling, if later useful, is specific, culturally workable, and secondary to the acute safety problem.

Questions about sexual health, injection drug use, and HIV are asked privately and neutrally. So are questions about travel and tick contact, and only because they can change testing and therapy. Consent and confidentiality matter. A person's identity is not used as a laboratory test.

Mobility and access are part of safe care. Crutches may be unusable for someone with upper-limb disability or poor balance. Stairs, bathroom access, and work are assessed before discharge. So are caregiving, transportation, rehabilitation coverage, and ability to reach follow-up. A professional interpreter and teach-back are used when needed. Shared decisions apply to reasonable drainage and rehabilitation options, but not to withholding urgent sepsis care without an informed goals-based reason.

Follow-up and contingencies#

During admission, the team follows fever, blood pressure, and pain. It follows swelling, wound or drain findings, and range of motion. It follows neurovascular status, kidney function, and blood count. It follows the inflammatory-marker trend and every culture. A falling C-reactive protein supports response but cannot replace clinical assessment. Persistent drainage, fever, bacteremia, or poor motion prompts a search. The search looks for inadequate washout, adjacent osteomyelitis, or abscess. It looks for resistant organism, endocarditis when indicated, or an incorrect diagnosis.

The discharge summary names the organism, susceptibilities, and drainage procedure. It names remaining culture status, the antimicrobial plan and monitoring, and kidney constraints. It names the mobility plan and the services responsible for follow-up. Physical therapy progresses motion and strength without forcing an inflamed postoperative joint. Return-to-work and driving advice reflects function and medicine effects.

Four future branches are preserved:

Culture ownership is explicit until final. A preliminary negative result is not presented as final, and an automated portal notification does not replace clinician contact when a culture becomes positive.

Reasoning traps and alternative pathways#

An alternative pathway would emerge if ultrasound localized inflammation to a prepatellar bursa with comfortable passive knee motion; that would require bursal rather than joint reasoning. A bloody aspirate in a patient receiving anticoagulation would elevate hemarthrosis but would not automatically cancel culture. Migratory pain, tenosynovitis, and pustular lesions would raise disseminated gonococcal infection. A large less-painful effusion after time in a Lyme-endemic region would support guideline-directed serum testing while bacterial septic arthritis remained proportionate to the acute findings.

Evidence limits and what could change#

Adult native-joint septic arthritis has a limited randomized evidence base, and recommendations often rely on observational studies and expert consensus. The 2023 SANJO guideline offers a current multidisciplinary framework, while the British Orthopaedic Association standard emphasizes time-sensitive multidisciplinary care. Local laboratory methods, resistance patterns, surgical capacity, and specialist availability still determine operations.

No synovial white-cell cutoff works as an absolute boundary. Immune suppression, prior antimicrobials, and organism can shift the count and culture yield. So can joint, sample quality, and timing. Gram stain is specific when an organism is seen but insensitive when none is seen. Crystal identification also depends on collection, microscopy, and reader experience.

The 2020 ACR gout guideline addresses gout management, not the complete emergency differential of every hot joint. CDC gonococcal guidance and the AAN, ACR, and IDSA Lyme guideline apply only when the epidemiology and syndrome fit. Importing those tests into every knee evaluation would produce false positives and distract from aspiration and bacterial culture.

The patient in this analysis has simultaneous calcium pyrophosphate crystals and Staphylococcus aureus septic arthritis. Coexistence is possible but is not the most common outcome. Another patient could have pure crystal disease, culture-negative infection, or hemarthrosis. Another could have reactive arthritis, Lyme arthritis, or a periarticular process. The safe conclusion comes from staged evidence and response, not from making one dramatic result carry the entire case.

Questions and answers

Do crystals in synovial fluid rule out septic arthritis?

No. Crystal arthritis and joint infection can coexist, so culture and the full clinical course still matter.

Is one synovial white-cell cutoff diagnostic?

No. Higher counts increase concern, but no single threshold confirms or excludes infection across all patients and immune states.

Should antibiotics always wait for aspiration?

In a stable patient, prompt aspiration before empirical therapy improves culture yield. Sepsis or shock requires immediate treatment and resuscitation without a dangerous delay.

Does a normal serum urate level rule out gout?

No. Serum urate can be within the reference interval during a flare and does not establish or exclude the cause of one hot joint.

Is an intra-articular steroid injection safe for presumed crystal arthritis?

Not while infection remains a meaningful possibility. A steroid injection can obscure or worsen an untreated joint infection and requires a verified diagnosis and procedural review.
