Renal angiomyolipoma#

What supports it. Visible macroscopic fat strongly supports classic angiomyolipoma, especially in the absence of calcification or invasive features.

What argues against it or keeps uncertainty open. Fat-poor variants can be difficult to distinguish, and a fat-containing renal cell carcinoma remains possible when other suspicious findings coexist.

Discriminating next step. Characterize fat and vascular anatomy, assess bleeding risk and syndromic context, and involve urology or interventional radiology when treatment thresholds are approached.

Renal infection, infarct, or pseudolesion#

What supports it. Fever, pyuria, wedge-shaped perfusion change, pain, embolic risk, or resolution on follow-up can indicate an inflammatory or vascular mimic.

What argues against it or keeps uncertainty open. A persistent rounded enhancing mass without compatible acute symptoms argues against a transient pseudolesion.

Discriminating next step. Correlate with symptoms, urinalysis, vascular risk, enhancement phases, and interval imaging rather than labeling an acute perfusion abnormality as tumor.

The differential is ranked but not closed. Probability, consequence of delay, reversibility, and test burden are considered together. A dangerous alternative can deserve early exclusion even when it is not the statistically most likely explanation.

Evidence-gathering strategy#

Tests are selected because they can change a decision, not because a broad panel feels comprehensive. Results are interpreted with their timing, pretest probability, measurement limitations, recent treatment, and the possibility that an apparently reassuring value was obtained too early or under the wrong conditions.

Progressive course and interpretation#

Prior images are retrieved and show that the lesion was present three years earlier but has enlarged slowly. Renal-protocol MRI demonstrates enhancement in a predominantly solid cortical mass without venous invasion or nodes. Slow growth lowers urgency but does not prove benignity. Equity and systems analysis#

Incidental findings are easily lost when imaging occurs in an emergency department, records sit in different health systems, or insurance authorization delays renal-protocol studies. Surveillance also assumes transport, time off work, and reliable reminders. Assign one team to track the next study, obtain outside images directly, provide language-concordant explanations, and offer lower-burden scheduling where clinically sound.
