Initial presentation#
A 30-year-old man requests an office visit after noticing a firm area in his right testis while showering, and he first felt it three weeks ago and thinks it has become easier to find. It is not painful. There has been no injury, fever, or dysuria. There has been no urethral discharge, scrotal redness, or nausea. There has been no vomiting, abdominal pain, or back pain.
He and his partner hope to have a biologically related child within the next two years. They have not tried to conceive, and he has never had semen testing. He asks whether an ultrasound or surgery could affect fertility. He also says that a genital examination makes him anxious because of a humiliating experience in adolescence; the clinician explains each part of the examination, offers a chaperone, and confirms that he can pause it at any time.
He has no history of undescended testis, infertility, or testicular tumor. There is no chemotherapy, pelvic radiation, or inguinal surgery. There is no sexually transmitted infection or family history of testicular cancer. He takes no medicine known to suppress gonadal function. He does not use anabolic-androgenic steroids. He reports no breast change, reduced libido, erectile difficulty, hot flashes, or loss of body hair. A private sexual and reproductive history is obtained without assuming partner anatomy, sexual orientation, or family-building method.
Vital signs are normal. The abdomen has no palpable mass. No supraclavicular or inguinal node is enlarged. The left testis feels normal. On the right, a firm, non-tender nodule is palpable within the upper half of the testis rather than separately in the epididymis. It does not transilluminate. There is no scrotal skin change, hydrocele, obvious varicocele, reducible inguinal bulge, or acute high-riding position. Cremasteric findings are not used as a stand-alone torsion test.
The primary-care clinician arranges urgent bilateral scrotal ultrasound with Doppler and calls the urology service to establish how a suspicious result will be handled, and the patient leaves with a named contact and instructions for sudden pain rather than a vague request to check the portal.
Problem representation#
This is a reproductive-age adult with a persistent, painless, firm nodule that appears intratesticular. There is no infectious, traumatic, or acute ischemic syndrome. He has no known risk factor, but lack of a risk factor does not make the lesion benign. Fertility and body-image priorities are important before a treatment decision. The system must also prevent loss of follow-up between ultrasound, markers, urology, pathology, and reproductive care.
The key distinction is not simply cancer versus no cancer. It is a solid intratesticular concern versus a benign extratesticular mimic, with an emergency torsion branch that would activate if the clinical story changed. The diagnostic pathway must move quickly enough for oncologic safety while preserving time for informed choices that do not cause a harmful delay.
Prioritized differential#
1. Testicular germ-cell tumor#
Reasoning for: A firm, persistent, painless intratesticular mass in this age group is concerning for a germ-cell tumor. Testicular cancer often presents without pain. The absence of constitutional symptoms or palpable nodes does not exclude localized disease.
Why it cannot be missed: Delay can permit progression and complicate treatment. Ultrasound, preoperative serum markers, inguinal surgical assessment, pathology, and stage-appropriate imaging are linked decisions. A normal marker panel or a small lesion cannot guarantee benign disease.
2. Benign epididymal or paratesticular lesion#
An epididymal cyst, spermatocele, adenomatoid tumor, or lipoma is often felt separately from the testis and may have characteristic ultrasound features; hydrocele surrounds the testis with fluid, while varicocele has a compressible venous pattern. These are common alternatives, but palpation alone can mislocalize a lesion.
3. Epididymitis, orchitis, or focal abscess#
Pain, tenderness, and swelling would support infection. So would urinary symptoms, fever, and pyuria. So would sexual infection risk and increased blood flow in a compatible pattern. This case lacks those findings. Empirical antibiotics without evidence could create a false interval of reassurance and delay repeat examination.
4. Testicular torsion or intermittent torsion#
The current painless three-week course is not typical of acute torsion. Even so, sudden severe unilateral pain, nausea, a new high position, or rapid swelling would create an emergency regardless of a previous benign-seeming visit. Doppler flow can support the diagnosis, but a convincing acute presentation should not wait for a delayed outpatient scan.
5. Inguinal hernia#
A reducible bulge connected to the inguinal canal, bowel sounds, or change with straining would support hernia. Incarceration or strangulation produces a different urgent pathway. No such finding is present, and the nodule appears within the testis.
6. Hematoma, infarction, granulomatous disease, or a non-germ-cell tumor#
Trauma, anticoagulation, and vascular disease can produce focal abnormalities. So can infection, inflammation, and rare testicular tumors. Ultrasound pattern, markers, serial change in genuinely indeterminate lesions, and pathology distinguish these branches. "Rare" does not mean impossible, and imaging cannot always provide final histology.
Focused history and examination#
The history clarifies when the finding was first noticed, whether it changes with position, and whether there has been pain, swelling, trauma, fever, urinary change, discharge, skin lesion, abdominal or back pain, weight change, breast symptoms, or prior episodes. Sudden severe pain is dated precisely because torsion decisions are time-sensitive.
Cancer risk questions include prior undescended testis or orchiopexy, personal or first-degree family history of testicular cancer, infertility, testicular atrophy, and a prior contralateral tumor; these modify probability but do not replace imaging. Testicular self-examination is not presented as population screening or as the patient's responsibility to prevent cancer.
Fertility history covers previous pregnancies with any partner, time trying to conceive, prior semen analysis, infections, pelvic or scrotal surgery, chemotherapy or radiation, erectile and ejaculatory function, testosterone or anabolic steroid use, and the person's goals. The clinician asks, "Would preserving the possibility of a biologically related child matter to you now or later?" rather than presuming the answer from age, relationship status, gender, sexuality, or existing children.
Examination is done in a warm, private room with consent and a chaperone according to preference and policy. It includes abdomen, groin, and nodes. It includes both testes, epididymides, cords, and scrotal skin. The clinician records size, location, and consistency. The record covers tenderness, ability to distinguish the lesion from the testis, and transillumination. It covers change with standing or straining and the contralateral testis. A gentle examination avoids repeated squeezing of a painful or frightening area.
Diagnostic strategy#
Localize the lesion promptly#
Bilateral high-frequency scrotal ultrasound with Doppler is the first imaging test for a newly palpable scrotal abnormality, and it can determine whether the finding is intra- or extratesticular, solid or cystic, and assess the contralateral testis and blood flow. Ultrasound features can raise or lower concern but do not deliver perfect histology.
Magnetic-resonance imaging is not a routine first test. It may help when ultrasound is inconclusive or when selected testis-sparing planning requires better local characterization. Computed tomography is not used to characterize the primary scrotal nodule. Adding advanced imaging before the ultrasound and urology decision can waste time, add cost, and create incidental findings.
Use markers for support and staging, not exclusion#
Serum alpha-fetoprotein, beta-human chorionic gonadotropin, and lactate dehydrogenase are measured before orchiectomy when testicular cancer is suspected. Elevation can support a germ-cell diagnosis, inform histologic reasoning, establish a staging baseline, and help later monitoring. Normal results do not exclude cancer. Pure seminoma does not produce alpha-fetoprotein, and conventional marker sensitivity is incomplete across tumor types and stages.
Complete blood count, kidney and liver measures, and other tests are selected for staging or treatment readiness, not because a broad laboratory panel can clear a solid mass. Infection testing is based on history, examination, urinalysis, and sexual sites rather than automatically attached to a testicular lump.
Protect the oncologic route#
A suspicious solid intratesticular lesion is referred promptly to urology. Standard diagnosis and initial treatment generally use radical inguinal orchiectomy, which supplies definitive pathology while avoiding a transscrotal route. Needle biopsy through the scrotum is not a routine shortcut when a germ-cell tumor is suspected because it can disrupt expected lymphatic pathways and increase local-management complexity.
Testis-sparing surgery is not a general alternative simply because fertility matters. It may be discussed in carefully selected small or indeterminate lesions, bilateral tumors, a solitary testis, or high likelihood of benign disease, with inguinal access, expert frozen-section support, informed acceptance of surveillance, and recognition that later orchiectomy may still be necessary.
Cross-sectional staging of chest, abdomen, and pelvis follows specialist assessment when malignancy is diagnosed or strongly suspected. Kidney function, contrast risk, and radiation burden determine timing and modality. So do symptoms, markers, and the probability of metastatic disease. Positron-emission tomography is not a routine initial staging substitute.
Start fertility planning before a deadline appears#
Fertility counseling begins when treatment is being planned, not after the final opportunity has passed. Sperm cryopreservation is an established option for postpubertal patients who can provide a sample. A reproductive specialist explains collection, semen analysis, and storage. The discussion covers costs, consent, disposition of stored material, and alternatives if an ejaculate sample cannot be obtained. The number and timing of samples depend on oncologic urgency and individual circumstances.
Baseline fertility may already be reduced in people with testicular germ-cell tumors. One normal contralateral testis often supports testosterone production and future fertility, but neither outcome is guaranteed. Semen results are not used to decide whether the mass is malignant.
Progressive results and interpretation#
Ultrasound the next morning shows a 2.1 cm solid hypoechoic intratesticular lesion with internal vascularity in the right testis. The epididymis and left testis appear normal, and there is no imaging pattern of torsion, abscess, hydrocele, or varicocele. The finding is concerning for a testicular neoplasm and is not suitable for an antibiotics-first trial.
Serum alpha-fetoprotein, beta-human chorionic gonadotropin, and lactate dehydrogenase are within their laboratory reference intervals. The clinician explains that this is useful baseline information but not a negative cancer test. The result lowers neither the urgency of urology review nor the weight of the solid ultrasound finding.
Urology sees him within days. Examination and imaging are concordant, the other testis is normal, and no circumstance makes a transscrotal biopsy appropriate. Staging imaging shows no enlarged retroperitoneal node or distant lesion. This makes localized disease plausible but cannot supply microscopic stage.
Before surgery, the patient meets a reproductive specialist. One semen sample shows a lower concentration than expected, which could predate the mass or relate to it. He chooses cryopreservation after reviewing storage, future use, and cost. The coordination is completed without postponing the planned oncologic procedure. He also discusses a testicular prosthesis and decides against placement at the initial operation, knowing that he can revisit the choice.
Radical inguinal orchiectomy is performed through the specialist pathway. Pathology confirms a seminoma confined to the testis without lymphovascular invasion in the examined specimen. Postoperative markers remain within the reference range. Final stage and recurrence-risk counseling integrate pathology, imaging, and marker kinetics; the word "confined" is not used as a promise of zero recurrence.
Management plan#
Before definitive diagnosis#
The primary-care role is to accelerate the correct sequence, manage distress, avoid delay-causing empirical treatment, and preserve questions for urology. Analgesia is offered if needed, although this mass is painless. The patient is advised not to repeatedly manipulate the lesion. Routine antibiotics, tumor-directed medicine, or a needle biopsy are not started.
The referral transmits exact location, duration, and examination. It transmits the ultrasound report and images, marker values and collection time, and fertility goals. It transmits contralateral testis findings, comorbidities, medicines, allergies, and a reliable contact method. The sender confirms receipt and an appointment rather than assuming the electronic queue is safe.
Surgery and staging choices#
For a suspicious mass with a normal contralateral testis, radical inguinal orchiectomy is the standard route. Risks include bleeding, infection, and anesthesia complications. They include pain, scar, and body-image effects. They include altered sensation and loss of the testis. Anticoagulants, antiplatelet medicines, diabetes, infection, and anesthesia risks are reconciled. No medicine or perioperative dose is specified here.
The patient is offered prosthesis counseling before surgery, including the option to defer. Consent separates removal of the affected testis from automatic permission for a prosthesis, fertility procedure, research use, or another intervention. Pathology review by a team familiar with testicular tumors matters because histology drives surveillance and adjuvant decisions.
After orchiectomy, management for stage I seminoma can include surveillance or selected adjuvant treatment. The choice considers exact pathology, ability and willingness to complete surveillance, radiation from repeated imaging, treatment adverse effects, recurrence risk, access, and the fact that rescue treatment is highly effective in properly monitored disease. A person who cannot reach repeated testing needs help solving the access problem, not automatic labeling as nonadherent.
Fertility, hormones, and sexual health#
Sperm cryopreservation is offered before cancer-directed therapy when feasible. The urgency of a rapidly progressing or life-threatening presentation can narrow the available time, but this stable localized case allows coordination. If semen collection is not possible or severe abnormalities are present, a reproductive-urology team discusses appropriate retrieval options. Experimental approaches are labeled as experimental.
The remaining testis often maintains testosterone and fertility, yet baseline function and later chemotherapy, radiation, or retroperitoneal surgery can alter outcomes. Symptoms of androgen deficiency prompt morning hormone assessment through the specialist pathway. Testosterone treatment is not started casually when near-term fertility matters because exogenous testosterone can suppress sperm production.
Sexual function, sensation, and erection are distinct outcomes. So are ejaculation, libido, fertility, and body image. The clinician asks about each rather than telling the patient he should feel fortunate or that one normal testis makes the issue trivial.
Escalation, referral, and safety net#
Sudden severe testicular or scrotal pain, nausea or vomiting, rapid swelling, a new high-riding testis, or pain after a transient similar episode requires emergency assessment for torsion. A known mass does not protect against a separate acute ischemic event. Fever, spreading redness, marked tenderness, hypotension, or systemic illness creates an urgent infection pathway.
New severe back or abdominal pain, shortness of breath, coughing blood, neurologic symptoms, rapidly enlarging nodes, or acute decline during the workup requires urgent reassessment for complications or advanced disease. These symptoms are not expected in this localized course described in this analysis, but the safety net names them.
Any solid intratesticular lesion, persistent indeterminate testicular finding, or concerning marker pattern requires expedited urology review. An ultrasound report marked suspicious is a clinician-to-clinician communication, not a routine patient message. If the appointment cannot occur within the planned interval, the primary-care team escalates to another service or center.
After surgery, emergency return thresholds include uncontrolled bleeding, fainting, and breathing difficulty. They include rapidly increasing groin or scrotal swelling, fever with worsening wound pain, inability to urinate, or severe uncontrolled pain. Routine postoperative questions use the urology contact rather than an emergency department when safe.
Communication, shared decisions, and equity#
The clinician uses plain uncertainty language: "The ultrasound shows a solid area inside the testis. That pattern can be cancer, although the scan alone cannot give final pathology. The blood markers are normal, but some testicular cancers do not raise them. Urology needs to assess this promptly."
Urgency is communicated without catastrophic phrasing. High cure rates are encouraging but do not erase fear, fertility concerns, or the need for timely care. The patient receives information in small sections, can invite a support person, and uses teach-back to explain the next three actions.
Anatomy, gender identity, sexual orientation, partner status, and desire for parenthood are not interchangeable. The team asks what terms the patient uses, who may receive information, and whether preservation of sperm or gonadal hormone function matters. A chaperone and trauma-informed examination are offered without treating anxiety as refusal of care.
Fertility preservation is often limited by cost, travel, and storage fees. It is limited by work schedules, language, and disability access. It is limited by religious considerations, immigration concerns, and short timelines. A patient navigator checks insurance, charity support, and transportation. The navigator checks interpreter needs, accessible collection facilities, and storage documents. Offering a brochure without confirming access is not a completed referral.
Body-image and prosthesis decisions belong to the patient. Neither accepting nor declining a prosthesis predicts coping. Mental-health support is offered when anxiety, depression, sleep disruption, or cancer-related distress interferes with daily function.
Follow-up and contingencies#
Before surgery, one clinician owns the ultrasound report, marker results, and urology appointment. That clinician owns the fertility referral, staging plan, and contact information. The patient receives copies but is not made the sole courier. A missed appointment triggers outreach and barrier assessment on the same working day.
After surgery, the team records the pathology date, reviewer, and exact histology. It records local extent, lymphovascular findings, and margin information where applicable. It records the pre- and postoperative marker trend and staging imaging. Urology or oncology discusses the result directly with the patient and documents the chosen surveillance or treatment plan.
Surveillance is a treatment strategy, not the absence of care. It requires an explicit calendar for history, examination, markers, and imaging, based on the final guideline and stage. It also requires a backup when travel, insurance, or relocation interrupts access. Repeated imaging carries cumulative radiation and incidental-finding risks, so studies are not added outside the stage-specific plan without a reason.
Fertility follow-up confirms storage status, fees, consent for future use, and the reproductive specialist's contact. Semen analysis can be revisited when the result would change family-building decisions. Hormone and sexual-health symptoms are assessed over time rather than assumed from a single postoperative value.
Four alternate result branches remain open:
- Ultrasound shows a clearly extratesticular cyst: Manage according to symptoms and imaging confidence, with reassessment if the examination and scan do not match.
- Ultrasound is indeterminate and markers are normal: Urology decides between short-interval imaging and inguinal exploration based on size, change, risk, and testicular context. Primary care does not choose an arbitrary delay.
- Markers are substantially elevated or staging suggests metastasis: Accelerate multidisciplinary oncologic care. Severe, life-threatening metastatic disease can alter whether systemic treatment precedes orchiectomy.
- The contralateral testis is abnormal or solitary function is at stake: Add endocrine and reproductive review, and discuss carefully selected testis-sparing approaches in an experienced center.
Reasoning traps and alternative pathways#
- Using absence of pain as reassurance: Testicular cancer often presents painlessly, while some benign and malignant lesions can hurt.
- Calling every scrotal lump testicular: Epididymal, cord, fluid, venous, and inguinal findings are common. Localization by examination and ultrasound matters.
- Waiting for tumor markers before arranging ultrasound or referral: Markers support diagnosis and staging but are not sensitive enough to clear a mass.
- Treating normal markers as no cancer: Seminoma and several early tumors may have normal conventional markers.
- Trying antibiotics without evidence of infection: Temporary delay can create false reassurance while a solid lesion persists.
- Ordering computed tomography before local ultrasound: Cross-sectional staging and local lesion characterization are different decisions.
- Using a transscrotal biopsy as a shortcut: The route can change lymphatic and local-management consequences when malignancy is suspected.
- Assuming fertility will be normal with one testis: Tumor biology, pretreatment semen quality, the other testis, and later therapy all matter.
- Delaying all treatment for multiple sperm samples: Preservation is important, but the plan must fit oncologic urgency.
- Discussing recurrence statistics without checking surveillance access: A strategy is safe only when the required follow-up can actually occur.
An alternative pathway with sudden pain and nausea would prioritize torsion and emergency exploration over outpatient cancer staging. Fever, pyuria, and epididymal tenderness would support infection while preserving repeat examination if a focal intratesticular finding remained. A reducible inguinal mass would redirect toward hernia care. A tiny nonpalpable incidental lesion with negative markers could enter a specialist-defined short-interval or testis-sparing discussion rather than automatic orchiectomy.
Evidence limits and what could change#
Current EAU and AUA guidance supports bilateral ultrasound, preoperative markers, inguinal management of a suspicious testicular tumor, stage-directed imaging, and early fertility discussion. Many details rely on observational evidence, expert consensus, and specialized-center outcomes. Management of small indeterminate lesions varies with ultrasound quality, pathology support, patient priorities, and regional practice.
Ultrasound is highly useful for localization but does not perfectly distinguish benign from malignant solid lesions. Serum alpha-fetoprotein, beta-human chorionic gonadotropin, and lactate dehydrogenase have important staging and monitoring roles but limited sensitivity. Emerging biomarkers are not yet a universal replacement for current diagnosis and pathology.
The 2025 ASCO update supports fertility counseling at diagnosis and offering sperm cryopreservation before cancer-directed treatment. Evidence about individual future fertility is less precise. Semen quality may already be reduced before treatment, and recovery varies by baseline function and subsequent therapy. Costs and access also shape what is feasible.
This case analysis ends with localized seminoma. A similar mass could be nonseminoma, a sex cord-stromal tumor, or a benign lesion. It could be infarction, infection, or a metastatic presentation. This high-risk case has completed human review by Jasaman (Jasmin) Tojjar, MD, PhD.

Questions and answers
Is a painless testicular lump less urgent than a painful one?
No. Pain does not reliably separate benign from malignant conditions. A persistent solid testicular finding needs prompt examination and ultrasound.
Do normal AFP, beta-hCG, and LDH levels rule out testicular cancer?
No. Some germ-cell tumors do not elevate conventional serum markers, so normal values cannot overrule a suspicious examination or ultrasound.
Should a suspicious testicular mass be biopsied through the scrotum?
Usually no. When malignancy is suspected, the standard diagnostic and treatment pathway uses an inguinal approach because a transscrotal route can alter lymphatic drainage and local recurrence risk.
Can fertility preservation wait until after treatment?
That depends on the treatment and urgency. Fertility goals should be discussed early, and sperm cryopreservation is offered before cancer-directed therapy when feasible without an unsafe delay.
Does removal of one testis always cause infertility or low testosterone?
No, but neither normal fertility nor normal hormone function can be guaranteed. Baseline function, the other testis, tumor effects, and later therapy all matter.
